- Opening Cases.
- Supervising field staff.
- Ongoing client assessments every 30-60 days.
- Client and employee education.
- Communicating with clients and their families with issues/changes regarding care.
- Escorting clients to medical appointments when needed.
- Medication Management.
- Clients live primarily in Essex, Morris, Union, and Passaic Counties. There may be some additional travel to other counties as needed. COMPANY CAR OR GAS REIMBURSEMENT PROVIDED.
- Current NJ license needed.
- Relevant Home Care experience.
