Case Manager Duties and Responsibilities
***:The FRSP Case Manager is responsible for providing home-based case management services for families experiencing homelessness who are rehoused in apartments throughout the District of Columbia. The primary goal is to assist families in addressing the barriers that would prevent them from maintaining permanent housing by utilizing the strength-based family first housing approach.


The functions performed will include:

family goal planning, connection to community resources, home visits, monitoring rental payments, landlord tenant engagement, and problem solving with other Case Managers.

Case management responsibilities may also include housing location.

This work is accomplished by advising and motivating program participants to accomplish all goals related to their permanent housing stability and Service Plan.


ESSENTIAL FUNCTIONS:


  • Case Manager will coordinate with Housing team to locate housing options for families.
  • Develop and update Family Goal Plans with the other program participants using a family holistic case management approach, assisting them to set and prioritize short and long term goals.
  • Complete monthly home visits to support families in performing the task required to maintain housing stability, including ontime payment of rent and household maintenance.
  • Conduct regular family assessments with a standardized tool for best practices.
  • Assist the families with resolving any landlord tenant issues as needed.
  • Provide families with referrals as needed to appropriate supportive services (Clinical and Family Holistic approach Programs) and work collaboratively as a team to help the family advance and reach short and long term goals.
  • Maintain current records in the Homeless Management Information System (HMIS) data base and other client referred case files, documenting interactions with families in the standardized format approved for best practices.
  • Capture daily and monthly statistics and other required reports as directed.
  • Maintain effective working relationships with other agencies to which (and from which) participants may be referred, regularly contributing to a resource directory/database.
  • Participate in special events, helping to share information and working as needed to ensure that every FRSP event is successful.
  • Attend all Staff meetings and mandatory trainings as required by the supervisor.

EDUCATIONAL REQUIRMENTS:

APPLICABLE EXPERIENCE, KNOWLEDGE, AND SKILLS.

  • Experience working with families, homeless, mental health, and/or substance abuse that reflect an understanding of the causes of homelessness is strongly preferred.
  • Working knowledge of the strengthsbased, client centered holistic approach.
  • Demonstrated cultural competence.
  • Excellent interpersonal and conflict resolution skills.
  • Excellent listening and problemsolving skills.
  • Strong oral and written communication skills.
  • Knowledge of community resources available to families.
  • Strong organizational skills.
  • Ability to be flexible, adapt and adjust to rapid change within a fast paced and growthoriented environment.
  • High confidence level in working independently.
  • Possess and maintain a positive, solutionoriented approach.

WORK ENVIRONMENT/PHYSICAL EFFORT

  • Must be willing to travel to different locations throughout Washington DC and perform home visits in any neighborhood.
  • Requires the ability to sit up to 36 hours per day with intermittent occasional walking and standing.
  • Office hours are 9am5pm, but case managers are expected to adjust their schedules to accommodate family case management and perform visits on evenings and weekends as needed.
  • Must be available to participate in after hours oncall rotation, as well as be on call during inclement weather

OTHER REQUIREMENTS:

Basic computer literacy and strong communication skills.

Must have sensitivity toward the program's target population and ability to comfortably and effectively relate to individuals of diverse backgrounds, and be respectful of cultural differences.

Must be responsible, organized, non-judgmental, able to work independently and as part of a team.

Employment Specialist must have a strong commitment to personal and professional ethics and standards and be able to set and maintain appropriate boundaries.

Pay:
$50, $60,000.00 per year


Schedule:

  • 8 hour shift
  • Monday to Friday
  • On call
  • Weekends as needed

Willingness to travel:

  • 50% (preferred)
