In Saxapahaw, NC, the Department III, Department of Social Affairs in the district of Spree-Neiße/Wokrejs Sprjewja-Nysa is seeking to fill a position as a Case Manager for Participation Planning/SPMD (m/f/d) starting on July 27, 2023, to cover for an employee during their maternity leave and potentially extended parental leave.

The main responsibilities of this position include:

Providing personal counseling in initial meetings, discussing suitable support options and possibilities within the framework of integration assistance based on the Social Code IX
Planning and coordinating assistance within the framework of Social Code IX and XII
Accepting applications for services within the scope of integration assistance according to Sections 90 ff of the Social Code IX
Special services for selfdetermined living for people with disabilities (integration assistance law)
Assessing the need for assistance, taking into account the individual's living situation and disabilityspecific circumstances using the Integrated Assistance Plan (IHP Brandenburg)
Preparing social histories and socialpedagogical opinions
Requesting and evaluating medical and therapeutic reports, if necessary contacting treating physicians or the Department of Health
Making decisions regarding institutional and financial support
Convening, preparing, following up, and conducting case conferences as needed
Creating overall plans according to Sections 117 ff of the Social Code IX / creating participation plans according to Sections 19 ff of the Social Code IX to document and manage the
