Currently seeking passionate individuals in all of Sacramento County for all positions (i.e. part-time, full-time, and per diem)

Job Summary

Responsibilities:

Conduct patient assessments and record medical histories.
Administer medications and perform treatments as prescribed by physicians.
Provide wound care and other nursing procedures as required.
Educate patients and their families on disease management, treatment plans, and selfcare techniques.
Monitor patients' progress and communicate any changes to the healthcare team.
Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.
Document patient care accurately and maintain confidentiality of patient information.
Provide emotional support and counseling to patients and their families to promote wellbeing and coping strategies.
Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.

Licensed Vocational Nurse Qualifications:

Current Licensed Vocational Nurse (LVN) license in the state of CA.
12 years of clinical nursing experience, preferably in home health, hospice, or a related setting.
BLS/CPR certification required.
Valid CA Driver's license.
