MetroCare of Springfield is a leading Adult Foster Care and Group Adult Foster Care provider.

Our nurses are an integral part of our care team, they are the bridge between a client's medical provider and caregiver.

We are searching for a passionate nurse to coordinate and advocate for our clients' care.


Over the course of 4-6 weeks, you will:

  • Learn the ins and outs of LPN case management.
  • Participate in weekly workshops that will cultivate your nursing skills. These workshops are tailored towards your specific goals and needs.
  • Have a mentor that will offer support and be a direct resource to you, even after you have graduated from the program

Eligibility

  • LPN must be fully licensed by the Massachusetts Board of Registration.
  • 2 years of recent experience in the direct care of the geriatric population or adults with disabilities (Clinicals, nursing/medical assistant, internship experience accepted)
  • Proficient computer literacy
  • This wonderful opportunity is a case managementfocused, handsoff position

Responsibilities:


  • Conducting and coordinating all applicable clinical assessments and clinical evaluations.
  • Visits must be completed in the qualified setting.
  • Completing documentation corresponding with each onsite visit or encounter.
  • Examining each client's AFC plan of care semiannually.
  • Selecting, training, evaluating, and managing AFC caregivers in conjunction with the case manager.
  • Providing ongoing training for AFC caregivers.
  • Monitoring the health status of all clients to ensure that they are receiving the best care possible and reporting any changes to the client's PCP.
  • Reviewing and updating client medications and diagnoses.
  • Planning for and implementing discharges and transition from the AFC program.

Compensation and Benefits:


  • Hours are 8am4:30pm, no weekends

Pay:
$ $37.00 per hour

Expected hours: 40 per week
