***: The Case Worker (CW) will ensure that eligible customers are offered quality services and strive to establish and maintain a positive rapport with each. He/she will provide services available through HCCAA funded programs or other community resources. The CW is expected to be knowledgeable about all funded programs offered and the CAP60 Data Base program. He/she will identify individuals or families eligible for CSBG Case Management services and actively work toward transitioning these families out of poverty.

ESSENTIAL JOB FUNCTIONS:
  • Become familiar with rules and guidelines pertaining to services provided through various programs
  • Become familiar with area resources in order to provide referrals
  • Serve as an advocate for the customer
  • Complete the Basic Intake Form to initiate all services
  • Set up each new client's profile in the data base software (Currently CAP60)
  • Update client's information in the data base software following each visit
  • Provide services within the framework of ROMA (Results Oriented Management and Accountability)
  • Enter personal time worked in the Automated Time Keeping system (Currently Employee Web Services)
  • Attend staff meetings, workshops and training events as required
  • Perform other tasks as deemed necessary

  • Identify potential case management clients and enroll them in the case management program
  • Work toward transitioning clients from poverty, ensuring that individual needs are met.
  • Conduct preassessment of potential clients through a formal interview to establish a service delivery plan for transitioning the client out of poverty
  • Coordinate with other agencies to ensure the accomplishment of individual goals; identify any preexisting family plans in effect with other agencies
  • Enter units of outcomes into the data base software under the ROMA standard
  • Document progress in the individual case management file/software on a monthly basis
  • Complete and submit on a timely basis the CSBG Monthly Client Service Reports and CSBG Case Management Reports
  • Energy Assistance_
  • Obligate energy funds for payment on client's utility bills
  • Maintain record of obligated funds vs. budgeted funds; report balances as needed
  • Weatherization_
  • Complete the Basic Intake form including Weatherization pages
  • Obtain appropriate income documentation
  • Assist Weatherization staff in locating clients' homes

JOB REQUIREMENTS:


EDUCATION/EXPERIENCE:
Minimum: High School diploma or GED


Preferred education:

Associate's or Bachelor's in Social Work or in a related field with experience in service delivery to the underprivileged or experience in HCCAA Programs.

Knowledge of interviewing techniques and customer sensitivity required.

SKILLS AND ABILITIES:Ability to work as a team member. Ability to communicate effectively, maintaining courtesy at all times. Ability to be respectful of and work effectively with individuals from various ethnic, socioeconomic, educational, and cultural backgrounds. Be flexible and have well-developed interpersonal skills. Be able to exercise independent judgment on a daily basis and maintain records and clearly understand written or verbal instruction. Ability to learn computer software as needed.


OTHER:
Professional in manner and attire. No record of a conviction which would contraindicate employment. Must have a valid Texas driver's license and a reliable vehicle with current vehicle liability insurance. Non-judgmental, unbiased attitude; must comply with all HCCAA Policies and maintain confidentiality in all phases of agency operations.

Computer, fax machine, 10-key calculator, copier, and telephone.

TYPICAL PHYSICAL DEMANDS:Requires sitting, standing, bending, reaching, and driving. May require lifting up to 40 pounds; requires manual dexterity sufficient to operate standard office machines such as computers, fax machines, calculators, telephones, and other office equipment; requires normal range of hearing/vision.


TYPICAL MENTAL DEMANDS:

Must handle new and diverse work problems on a daily basis. Personal maturity is an important attribute. Must be able to resolve problems, handle conflict, and make effective decisions under pressure.

Must have the ability to listen objectively to people, perceive the real problem and assist in bringing issues to a successful conclusion.

Must relate and interact with employees at all levels within the Agency. Bilingual skills are encouraged.


WORKING CONDITIONS:

Occasionally called upon to work outside of the normal 8-5 work day. Travel to staff meetings or workshops as required. All information associated with the Agency is confidential.
