- Establishes initial and on-going therapeutic relationship with Cornerstone clients needing mental health counseling.
- Communicates results of assessment to staff involved in clients' care.
- Participates in development of an initial plan of care to address prioritized needs of client.
- Provides ongoing services as prescribed on plan of care.
- Makes referrals for additional services as needed.
- Assists clients in securing external behavioral health resources.
- Engages with community resources to ensure client is receiving necessary services.

Description
Helping Up Mission is seeking a COMental Health Therapist who will provide counseling under the oversight of the Associate Clinical Director, and in the context of a behavioral health team lead by a psychiatrist. The Mental Health Therapist will be responsible for the delivery of mental health counseling in group and individual setting to homeless persons with mental health disorder to include substance use. These persons are housed at Helping Up Mission. Additionally, this position will work with physicians, mid-levels, primary care providers, and other specialists to promote qualitative and efficient use of health care services.
