- Provide culturally sensitive, linguistically appropriate assessments following NASW code of ethics.
- Conduct bio-psychosocial assessment interviews with patients according to department policy and procedures.
- Formulate treatment plans and engage with patients to improve behavioral health conditions.
- Attend clinical supervision sessions and monthly case consultations.
- Offer crisis intervention/treatment to adult patients in emergent situations.
- Maintain a regular case load and provide Warm Handoffs between primary care providers and patients.
- Document treatments and progress following state regulations and department procedures.
- Support and counsel patients with psychoeducation.
- Submit billing records timely for behavioral health services.
- Perform other job duties as required.
- Hold a MSW from an accredited school of social work, ASW required.
- Experience with one-on-one patient health.
- Excellent written, verbal, and interpersonal communication skills.
- Interest in integrating primary care and behavioral health care.
- Current documentation of Basic Life Support is required.
- Must be fluent in English and Chinese (Cantonese and/or Mandarin).
- Proficiency in other Chinese dialects or languages is an asset.

We are looking for an ACSW/AMFT to join our team! As an ACSW/AMFT, you will be responsible for providing assessment, diagnosis, treatment, crisis intervention, case management for members with mild to moderate behavioral health needs. You will formulate treatment plans, conduct psychotherapy-treatment, and refer members with acute or chronic psychiatric and/or behavioral health needs. Collaborating with treating physicians is key to plan and direct each patient's treatment program through a collaborative care model.
