Position Summary:


The
Family Assessment Worker identifies and confers with families of newborns, in hospital or post-discharge in Orange County, to determine their stress level, utilizing the Healthy Families Florida Assessment Tool (HFFAT).

Responsibilities:


Essential Functions

  • Based upon information from families, makes referrals to the Healthy Families Initiative and/or other appropriate family support programs in the community.
  • Establishes and coordinates a system to obtain referrals of pregnant women or those who have just given birth, from all major hospitals, birthing centers, Orange and/or Osceola County Healthy Start Coalition, OB-GYNs and other pre
- and post-natal establishments in the Orlando area.

  • Initiates contact with prospective participants in the Healthy Families Initiative, in the hospital or at home.
  • Administers the HFFAT with all families.
  • Scores and analyzes HFFAT results; encourages families to participate in the Healthy Families Initiative when appropriate; provides families with program overviews; counsels families regarding community resources that may benefit them; and provides information to Healthy Families program staff regarding families referred for long term homebased support.
  • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
  • Maintains compliance with all Orlando Health policies and procedures.

Other Related Functions

  • Participates in appropriate inservice training sessions and team meetings.
  • Attends community meetings and/or functions.
  • Works closely with hospitals/OB-GYNs to ensure a 100% rate of referral to the program.
  • Tracks down referred families, even after hospital discharge, including visiting families in their homes if necessary.
  • Documents activities and generates and reports programmatic data.
  • Participates with the team in program planning and development, defining and implementing programmatic objectives, procedures and policies, and ensuring the continuous improvement of services.
  • Works with families in a manner that encourages them to make their own choices; responds to the needs and priorities they have established; respects their right to have personal and confidential matters dealt with in a dignified manner; and esteems their ethnic and cultural distinctiveness.
  • Provides comprehensive, regular updates to the Program Manager on the status of programmatic activity.

Qualifications:

Education/Training

  • Bachelor's degree in Child Development, Psychology, Social Work, Nursing, Education or a related field is required; or
  • Associates of Art (A.A.) degree plus two (2) years' experience working with families; or
  • Child Development Associate (CDA) plus three (3) years' experience working with families; or
  • High School (H.S.) diploma or General Education Development (GED) plus six (6) years' experience working in a home visiting program.

Licensure/Certification

Valid Florida driver's license with no more than six (6) points, at least 21 years of age, and must be insurable under the Orlando Health insurance carrier's requirements and standards.

Access to automobile, and ability to travel to various sites throughout the community.


Experience

  • Requires at least one (1) year of experience working in the areas of early intervention, social work, counseling, maternal/childcare, child abuse and neglect, and/or family support.
  • Experience in working with families of similar cultural background to program participants.
  • Thorough knowledge of family support services in the Orlando area.
  • Acceptance of individual differences, knowledge of infant and child development, open to reflective practice (i.e. has capacity for introspection, communicates awareness of self in relation to others, recognizes value of supervision, etc.)
