- Assist in documenting in Recovery Services Case Management System (RSCMS) including treatment planning, case review, and progress notes documentation following Recovery Services policies and procedures.
- Assist in completing all documentation into the Recovery Services Case Management System (RSCMS).
- Maintain a relationship with all partnering agencies and referral sources.
- Assist coordination with referral sources, family members, community agencies, and other treatment providers to provide the best treatment and case management options for participants.
- Conduct follow-up on referrals as directed.
- Observe case-staffing sessions that ensure continuity-of-care addressing progress in case management goals and objectives.
- Observe and participate in group and individual sessions.
- Assist in conducting participant Urine Analysis (UA) and Breath Alcohol Concentration (BrAC) testing.
- Attend training and direct support meetings.
- Work collaboratively with clinical team members and attend case conferencing.
- Participate in training in areas related to the improvement of service delivery.
- Attend staff meetings and works collaboratively with staff in case-staffing and review.
- Coordinate the transport of participants on assigned activities, case management services, and outings.
- Maintain a respectful relationship with participants, former participants, family members, and peers.
- Assist in maintaining, tracking, and documenting all pertinent information for monthly and quarterly reports.
- Participate in community meetings as requested.

- Conduct follow-up on all referrals.
- Assist participants, families, and other sources to aid in diagnoses and the development of individual treatment plans.

- Support participants with participant's timely medication refills and self-administration of medication according to policies and procedures.

General Functions:
Case Managers provide comprehensive case management services to participants involved across Recovery Services' Continuum of Care. This position focuses on connecting participants to services and teaching life-skills. The Case Manager utilizes a strengths-based approach to engage participants in the development of pre-and post-treatment planning.
