    JOB DESCRIPTION

    Job Title: Case Manager I/II/III/IV

    Location: Chugiak, AK - Ernie Turner Center

    Department: Recovery Services

    Reports To: Assigned Supervisor

    Supervises: None

    FLSA Status: Non-Exempt

    Pay Grade: N4 (I/II), N5 (III/IV)

    Job Type: Regular, Full-Time

    General Functions:

    Case Managers provide comprehensive case management services to participants involved across Recovery Services' Continuum of Care. This position focuses on connecting participants to services and teaching life-skills. The Case Manager utilizes a strengths-based approach to engage participants in the development of pre-and post-treatment planning.

    Duties and Responsibilities:

    Case Manager, I

    • Assist in documenting in Recovery Services Case Management System (RSCMS) including treatment planning, case review, and progress notes documentation following Recovery Services policies and procedures.
    • Assist in completing all documentation into the Recovery Services Case Management System (RSCMS).
    • Maintain a relationship with all partnering agencies and referral sources.
    • Assist coordination with referral sources, family members, community agencies, and other treatment providers to provide the best treatment and case management options for participants.
    • Conduct follow-up on referrals as directed.
    • Observe case-staffing sessions that ensure continuity-of-care addressing progress in case management goals and objectives.
    • Observe and participate in group and individual sessions.
    • Assist in conducting participant Urine Analysis (UA) and Breath Alcohol Concentration (BrAC) testing.
    • Attend training and direct support meetings.
    • Work collaboratively with clinical team members and attend case conferencing.
    • Participate in training in areas related to the improvement of service delivery.
    • Attend staff meetings and works collaboratively with staff in case-staffing and review.
    • Coordinate the transport of participants on assigned activities, case management services, and outings.
    • Maintain a respectful relationship with participants, former participants, family members, and peers.
    • Assist in maintaining, tracking, and documenting all pertinent information for monthly and quarterly reports.
    • Participate in community meetings as requested.
    • Other duties as needed and assigned.

    Case Manager, II

    • Perform all duties and responsibilities of the Case Manager, I position with a higher level of proficiency.
    • Support Case Manager, I staff by answering questions, providing guidance, and making recommendations related to the Case Manager, I's position's duties and responsibilities.
    • Document in Recovery Services Case Management System (RSCMS) including treatment planning, case review, and progress notes documentation following Recovery Services policies and procedures.
    • Complete all documentation into the Recovery Services Case Management System (RSCMS).
    • Maintain a relationship with all partnering agencies and referral sources.
    • Coordinate with referral sources, family members, community agencies, and other treatment providers to provide the best treatment and case management options for participants.
    • Understand and use Trauma Competent and Motivational Interviewing (MI) techniques to encourage participants to meet their clinical goals.
    • Meet and assist participants in identifying case management services, individualizing treatment plans following best practices, and coordinating employment, mental health, housing, relapse prevention, and other related referrals.
    • Conduct follow-up on all referrals.
    • Assist participants, families, and other sources to aid in diagnoses and the development of individual treatment plans.
    • Participate in case-staffing sessions to ensure continuity-of-care addressing progress in case management goals and objectives.
    • Participate in group and individual sessions.
    • Support participants with participant's timely medication refills and self-administration of medication according to policies and procedures.
    • Conduct participant Urine Analysis (UA) and Breath Alcohol Concentration (BrAC) testing.
    • Attend training and direct support meetings.
    • Work collaboratively with clinical team members and participates in case conferencing.
    • Participate in training in areas related to the improvement of service delivery.
    • Attend staff meetings and works collaboratively with staff in case-staffing and review.
    • Transport participants on assigned activities, case management services, and outings.
    • Maintain a respectful relationship with participants, former participants, family members, and peers.

    Case Manager, III

    • Perform all duties and responsibilities of the Case Manager, I and II position with a higher level of proficiency.
    • Support Case Manager, I and II staff by answering questions, providing guidance, and making recommendations related to the Case Manager, I and II's positions' duties and responsibilities.

    Case Manager, IV

    • Perform all duties and responsibilities of the Case Manager I, II and III positions with an advanced proficiency level.
    • Support Case Manager I, II and III staff by answering questions, providing guidance, making recommendations, and conduct case management training related to Case Manager I, II and III's duties and responsibilities.

    Internal / External Relations:

    • Maintain a positive working relationship with CITC staff and personnel of other partnering organizations.
    • Maintain confidentiality at all times while displaying a professional disposition.

    Job Responsibilities Related to Participant Privacy

    • Employee is expected to actively participate in CITC privacy training and to protect the privacy of participant information in accordance with CITC's privacy policies, procedures and practices, as required by federal and state law. Failure to comply with CITC's policies and procedures on participant privacy may result in disciplinary action up to and including termination of employment.
    • Employee may access protected health information and other participant information only to the extent necessary to complete job duties. Employee may only share such information on a need to know basis with others who have job responsibilities related to treatment, payment, or other CITC operations.
    • Employee is encouraged and expected to report, without the threat of retaliation, any concerns regarding CITC's policies and procedures on participant privacy and any observed practices in violation of the policy to the designated Privacy Officer.

    • Additional Job Specifications for Case Manager, IV:
    • Demonstrated working knowledge of ASAM and DSM-5 diagnostic tools.
    • Demonstrated working knowledge of the State and Government clinical regulations.

    Minimum Qualifications:

    Case Manager, I

    • High School Diploma or GED.
    • Alaska Chemical Dependency Counselor Certification I (CDC), or can obtain certification within one (1) year from the date of hire.

    Case Manager, II

    • High School Diploma or GED.
    • Two (2) years of experience working in a related social services field, as stated in the general functions.
    • Alaska Chemical Dependency Counselor Certification I (CDC), or can obtain certification within six months from the date of hire.

    Case Manager, III

    • Associates degree in Human Services or related field. Relevant experience may qualify for the education requirement on a year-for-year basis.
    • Four (4) years of experience working in a related social services field, as stated in the general functions.
    • Alaska Chemical Dependency Counselor Certification I (CDC), or the ability to obtain certification within three months from the date of hire.
    • Working knowledge of ASAM and DSM-5 diagnostic tools.

    Case Manager, IV

    • Bachelor's Degree in social work, psychology, human services, counseling, or substance abuse or addiction counseling. Relevant experience may qualify for the education requirement on a year-for-year basis.
    • Six (6) years of direct experience working in the substance use field with experience implementing and facilitating clinical best practices.
    • Alaska Chemical Dependency Counselor Certification II (CDC) or higher, or the ability to obtain certification within three (3) months from date of hire.
    • Working knowledge of ASAM and DSM-5 diagnostic tools.

    Minimum Qualifications at all Levels:

    • Continued employment is contingent upon completion of a satisfactory state and federal fingerprint criminal background check, approved National Provider Identifier Standard (NPI) status, and state approval as a Qualified Addictions Professional (QAP).
    • Valid Alaska driver's license and insurable under CITC's automotive insurance, which requires a driver to be at least 21 years of age and have had a driver's license for at least three years.

    Physical Requirements:

    • Primarily works in an office setting, with extended periods of time at a desk and on a computer.
