    Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Case Manager/Peer Support Specialist in Worcester, MA.


    The City of Worcester Crisis Response Team provides clinical support 7 days per week, 24 hours per day in partnership with the Worcester Public Service, and first responders in the city of Worcester.

    The initiative serves individuals experiencing a mental health and/or substance use disorder and other social concerns.

    The Crisis Response Case Managers provide all levels of support and coordination of essential services to individuals and families requesting services at Community Healthlink through the Crisis Response Initiative.

    This role requires an understanding of CHL's mission, vision, and core values. This position requires candidates to gain knowledge of CHL's service lines to appropriately triage.

    Responsibilities


    Position Responsibility:

    • The Peer Specialist (PS) will provide peer support services such as information, training and advocacy in order to assist Persons to regain control over their own lives and over their own recovery process.
    • The Peer Specialist will provide a unique perspective and related skills to the program and perform a wide range of tasks to assist Persons in increasing knowledge of and access to community resources, and partner with Persons the development of a plan to achieve personal growth.

    Essential Responsibilities:

    • Perform comprehensive case management intake assessment and develops plan with client for obtaining services needed.
    • Complete initial behavioral health intake documentation as required by the program, payor, and regulatory bodies.
    • Work collaboratively with the integrated team to coordinate the delivery of appropriate behavioral health and other support services, including coordinating supportive transitions in care providers.
    • Provide ongoing case management services to assist clients with obtaining various entitlements and benefits (i.e., SSI, EAEDC, MassHealth and other health insurance needs, addiction services, and other community resources).
    • Assist clients in accessing transportation through existing resources or accessing new resources.
    • Responsible for ensuring that after care needs of clients are met through provision of appropriate referrals for community-based services including substance use disorder, mental health, medical, legal, rehabilitative social & vocational services, advocacy, and coordination of care including ongoing coordination with Central Intake Coordinator around facilitation of after care service plans.
    • Provide outreach as designated to community-based residents.

    • The Peer Specialist will be able to work effectively and as part of a team to provide information, support, and advocacy to assist Persons in articulating and developing their recovery goals.
    • The Peer Specialist will be able to provide service in a manner that meets agency requirements and the needs of the Person.
    • The Peer Specialist will be able to effectively document service activities according to agency/program standard.
    • Assist, encourage and empower Persons using mental health and addiction services.
    • Facilitate mutual support and self-help groups.
    • Work with and support Persons to prepare for formal and informal meetings including IAP meetings and social networking events.
    • Build effective working relationships with agencies and organizations in support of the Person's recovery.
    • Participate in workshops, conferences and training to keep current with emerging evidenced based recovery-oriented practices.
    • Attend Integrated Staff Meetings and Daily Rounds.
    • Will provide transportation to persons as needed. May include transporting in individuals in personal or program vehicle .
    • Performs other related duties.

    Qualifications


    • Grade C02: Access Case Manager I: High school diploma/GED/HiSet required; Associates degree preferred.
    • Grade C04: Access Case Manager II: Requires a bachelor's degree; preferably in the field of psychology or human services .
    • Basic knowledge of CHL services and insurance requirements and reimbursement for these services.
    • Excellent oral communication and computer skills .
    • Working knowledge of Electronic Health Records (EHR).
    • Ability to multitask. Must possess time management skills with a strong sense of prioritzation and follow through.
    • Requires personal and professional accountability, self-management, and initiative.
    • Professional integrity and cultural competency.
    • Ability to use office equipment, including copy machines, computers, printers, telephones .
    • Ability to organize complex tasks and meet deadlines.
    • Current valid US-issued driver's license and registered, inspected, and insured automobile for work related purposes including transporting clients in a personal vehicle.
