Nurse Case Manager/Utilization Review

Overview:
We are seeking a dedicated and compassionate Nurse Case Manager to join our team.

As a Case Manager, you will be responsible for providing comprehensive care coordination and support to patients in a hospital setting.

Duties:


  • Conduct patient assessments and develop individualized care plans
  • Collaborate with healthcare professionals to ensure appropriate treatment plans
  • Monitor patient progress and adjust care plans as needed
  • Coordinate discharge planning and ensure smooth transitions of care
  • Advocate for patients' needs and rights
  • Maintain accurate and confidential medical records
  • Provide education and support to patients and their families
  • Comply with HIPAA regulations and maintain patient privacy

Requirements:


  • Bachelor's degree in nursing or related field (preferred but not required)
  • Previous experience in case management or related healthcare role
  • Strong knowledge of hospital procedures and protocols
  • Familiarity with ICU and ER settings
  • Excellent communication and interpersonal skills
  • Ability to multitask and prioritize responsibilities
  • Proficient in medical record documentation
  • Knowledge of patient assessment techniques and vital signs monitoring
