    Position Summary


    The Licensed Practical Nurse (LPN) working in the Inpatient setting provides bedside nursing care that requires the application of basic knowledge of biological, physical, behavioral, social and nursing sciences under the direction of the Registered Nurse (RN).


    Essential Job Functions/Accountabilities


    Contributes and properly documents the implementation, and evaluation of the communicated nursing plan of care that was established by the RN.

    Administration of medication and treatments within Ohio's LPN professional scope of practice. May administer adult intravenous therapy in accordance to ORC or


    Identifies significant changes and potential problems concerning patient/significant others and communicates those pertinent issues to the R.N or authorized directing health care provider.

    Contributes subjective and objective observations and health information to the nursing assessment. This includes completing physical assessments and obtaining patient health histories. Demonstrates competency of clinical skills that are established by CCOC's policies and procedures

    Assures that the privacy, comfort and safety needs of the patient are met. Provides individualized patient care by following the patient's plan of care that was created by the RN. Maintains up to date and accurate documentation of nursing care provided in the electronic health record. Provides education and information to patients and family members. Delegation of nursing tasks as directed by a RN. Provides orientation and guidance to new LPNs and ancillary staff as authorized by the RN

    Participates in the patient care team to achieve the organization's mission and purpose. Ensures Hourly Patient Rounding is incorporated in to the daily Patient Care routine. Seeks opportunities for professional and personal growth by attending educational programs, serving on committees and reading current professional journals. Consistently promotes customer satisfaction through positive behaviors when interacting with patients, visitors and fellow staff members.


    Supports other direct care givers as needed by assisting with multifunctional activities such as direct patient care and patient care support duties to include lifting, turning, repositioning and ambulating patients as well as environmental, dietary, transportation, supplies, light maintenance activities and administrative/clerical tasks.

    Records patient charges timely and accurately as described in Departmental Procedure Manual. Participates in ongoing evaluation to ensure quality patient care. Works well with patients, physicians and other CCOC employees.

    Successfully meets Unit Specific Skill Sets. Complies with all Standards of Behavior set forth by CCOC Follows HIPAA, OSHA, ODH rules, and Compliance Plan regulations

    All other duties not specifically assigned.


    Position Requirements

    Education:
    Graduate of a school of Practice Nursing.

    Experience:
    Three (3) to Five (5) years of experience in field or profession preferred


    Technical Skills:
    N/A


    Certifications/Licenses/Registrations:
    Current license to practice practical nursing in the State of Ohio.

    If licensed before 2/1/2022, authorized to administer medications by The State of Ohio Board of Nursing (LPN M-IV) is required as licensure certification.

    If licensed after 2/1/2022, transcripts from Practical Nursing program are required showing the LPN has taken relevant Meds/IV courses within their Practical Nursing program.

    BLS required within three (3) months of hire.


    Requirements

    Schedule: 3x 12-hour shifts per week; Night Shift 7PM-7:30AM

    Full-Time 36 hours per week
