- Develops and implements individual treatment plans for assigned patients
- Implements or modifies discharge planning
- Communicates with referral sources, insurance companies and managed care companies
- Documents patient progress verbally with staff and written in medical chart or as needed
- Participates in staff meetings, supervision and rounds
- Prepares and delivers lectures on chemical dependency
- Participates in crisis intervention
- Facilitates psychiatric evaluations and/or referrals
- Participates in any activity as directed by the Evaluation Supervisor
- Conducts assessment/evaluations as directed
- High School diploma or GED
- Minimum two years counseling experience in long term C.D. drug free program.
- Using a lens of equity in all aspects of patient care delivery, education, and research to promote policies and practices to allow opportunities for all to thrive and reach their potential.
- Serving as a role model confronting racist, sexist, or other inappropriate actions by speaking up, challenging exclusionary organizational practices, and standing side-by-side in support of colleagues who experience discrimination.
- Inspiring and fostering an environment of anti-racist behaviors among and between departments and co-workers.
