- Identifies, tracks and manages care for the highest risk patients.
- Conducts initial care coordination screening eligibility and performs comprehensive risk assessment.
- Works directly with patients/family members and providers to develop and implement an individualized and integrated patient care plan for each of the highest risk patients.
- Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans.
- Adapts practice to meet individual patient circumstances with regard to health literacy.
- Deficits as well as cultural and linguistic differences, ensuring that effective communication is achieved, particularly where there may be barriers to understanding.
- Utilizes motivational interviewing to gain cooperation from patients and families around the care management plan.
- Ongoing clinical assessment, monitoring and follow up for the high-risk patients.
- Utilize standing orders and clinical orders for chronic disease management within scope of clinical practice and per individual medical home policies and procedures.
- Assesses psychological needs and ability to benefit from community resources and referrals to outside agencies/programs when appropriate.
- ASN or BSN Registered Nurse
- 3-5 years of experience with 1-2 years years clinical nursing experience in disease management or care management in a clinical, acute care, or community setting. Knowledge of healthcare and managed care preferred.
- Bilingual (required): Ability to read, write, and speak proficiently using the both English and one of the following languages: Spanish, Arabic, Farsi, Nepali language.
- Knowledge of healthcare and managed care preferred.
- Prior experience working in a remote role is preferred.
- Prior case management experience preferred.
- Current Illinois licensure as a professional nurse (RN) both active and in good standing.

The RN Care Manager is responsible for coordinating screenings, assessments and for providing care management services to patients identified as having complex chronic care needs. Acts as connector between patient needs and the resources provided by the care management program.
The RN Care Manager will promote self-care management and assist with access to care. The care manager works with patients through direct face to face patient contact or telephonically. Provides guidance and leadership in the care management program to those in the healthcare organization as well as ensuring the patients receive the best possible care.
The RN reports to the Medical Director, Practice or Clinical Manager.
