RN Case Manager:

Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each participant's unique needs

Clinic RN:

Conduct comprehensive assessments, administer treatments, and provide health education to participants and their families

Enrollment RN:

Conduct thorough assessments to assess eligibility and guide seniors and their families through the enrollment process for our PACE program

Home Health RN:

Provide skilled nursing care to seniors, perform home assessments, develop personalized care plans, and coordinate services to meet the evolving needs of our participants

Locations throughout California:

Stockton, Modesto, Fresno, San Jose, Pasadena, Riverside/Coachella Valley, Long Beach, North Hollywood and Rosemead

RN Job Requirements:

Graduation from an accredited nursing program and a minimum of one (1) year of experience as an RN
Current and unencumbered RN license in California
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Prior experience in senior healthcare or PACE preferred, but not required
Competitive pay with

Salary/Wage base range for this role is $ $63.02 hourly + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and role.
