Compensation Range:
$65,000 - $73,000 /yr commensurate with experience.


Overview:

Scatter-site Health and Housing Improvement Project SHIP (70 units) is seeking a Program Coordinator to assist with program operations. The Program Coordinator (PC) is responsible for the supervision of the Residential Case Managers at 180 Christopher street, Bailey Holt House

  • Bailey house. The PC ensures care coordination of resident needs, quality assurance of the Housing programs, and direct service support. The PC ensures resident records are incompliance as per HMIS/CCoC HUD standards and other regulatory requirements, assists residents with entitlements, provides ongoing psychosocial support, monitors health and wellbeing, and makes appropriate referrals, both internally and externally, as needed.

Responsibilities:


  • Provide documented formal supervision to the Scattered site Residential Case Managers.
  • Manage quality assurance of the program through record checks and data reports.
  • Act as the main representative of the SHIP Programs, including participating in interdisciplinary meetings, treatment planning meetings, and special events.
  • Facilitate monthly clinical meetings
  • Meet with residents on an asneeded basis, while ensuring monthly home visits by staff and the direct provision of quality case management for all residents.
  • Ensure that the SHIP Housing sites are contract compliant, consistent with HUD/ NYC CCoC HMIS standards.
  • Monitor medical and mental health conditions, including viral suppression; of residents and make appropriate referrals, as needed.
  • Conduct intake meetings with new referrals and inform the Program Director and the clinical team of all "High Risk" behaviors or medical conditions.
  • Help new residents move into their apartments, including apartment setup.
  • Develop and maintain initial Assessments within 30Days, and Reassessments every sixmonths.
  • In collaboration with the client, create individual service plans that are strengthbased and clientcentric.
  • Appropriate interventions to reduce unnecessary decompensation, hospitalization, relapses and escorts clients/ and assist with home visits as needed.
  • Provide referrals and followups when necessary for substance use, nutrition, clothing, mental health, and dental; said referrals should be internal whenever practical and external as needed.
  • Complete all required reports and submit required data on or before the prescribed deadlines.
  • Plan, organize, and facilitate recreational activities.
  • Ability to teach basic Applied Daily Living Skills, including personal hygiene and grooming, meal preparation, laundry, housekeeping, and home safety.
  • Demonstrate proficiency with case management (eIcare and eShare) and medical (eClinical Works) software.
  • Ensure linkage to entitlements and maximize income potential through SSI, SSD, etc.
  • Participate in weekly formal supervision with your supervisor. Come to meetings prepared.
  • In coordination with your supervisor
- review monthly rent roll documentation and develop individual plans of action to address arrears.

  • Complete all trainings required by the agency.
  • Complete trainings that are required by your relevant profession or credential, including Narcan training on an annual basis. Encourage residents to participate in relevant offered trainings.
