Overview

Ebenezer offers a robust compensation package with competitive benefits and wages ranging from $29 - $46 per hour. All Wage determinations will be based on similar experience.
Shift Differential Available on Evenings and Nights

The Fountains at Hosanna is looking for a Registered Nurse - RN or an Licensed Practical Nurse - LPN whom would provide personal cares to improve and/or maintain the senior residents' physical and emotional well-being for our beautiful and active Assisted Living/Memory Care in Lakeville, MN.

The team includes a Director of Health Services Nurse, 3 other Licensed Nurses, Resident assistants trained to pass medications, as well as activities, culinary, housekeeping staff.

This full time nursing schedule includes;

72 hours per pay period
Days/Evenings (two evenings shifts 2pm-10:30pm every 4 weeks)
Weekends - every 4th
Holidays - every 4th

Responsibilities/Job Description

Monitors personal cares provided to assure consistency with each individual care plan
Coordinates medications and performs treatments per physician orders and facility policies
Assesses resident goals and revises, modify care plans as needed
Provides reports and documentation related to incidents, change of status and cares provided
Advise Resident Assistants as needed

Qualifications

Current MN RN license or LPN License by start date
AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers

Preferred:

Comfortable using Microsoft windows and electronic charting systems
Work experience in a patient support role, LPN or RN
Good written and verbal communication skills to engage with residents
