    For more than 28 years Pegasus has been a leader in our field. We are a preferred partner for Kaiser and Optum and several area hospitals. Therefore, you will always have as many visits as you desire.

    REQUIREMENTS:
    _

    • Availability of at least two days, or ten productivity units a week and one weekend a month for per diem. Visits are typically conducted between the hours of 8 am-5 pm.
    • Reliable transportation, current driver's license, auto insurance
    • At least one year experience with direct patient care
    • Able to perform basic wound care, blood draw, and catheter care (OR willingness to learn these skills)
    _ESSENTIAL JOB FUNCTIONS/


    RESPONSIBILITIES:
    _

    • Make home visits to clients in the community, as well as Senior Living, Assisted Living, and Independent Living facilities.
    • Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
    • Assess cognitive, psychological and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
    • Evaluate home environment for hazards or barriers to more independent living.
    • Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
    • Continually assess and revise the occupational therapy care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
    • Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living.
    • Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
    *Visits Requiring OASIS Documentation*$105/visit – Start of Care
    $ 80/visit – Recertification of Care
    $ 80/visit – Resumption of Care
    $ 80/visit – Discharge
    $ 20/visit – Non-Visit Discharge

    *Non-OASIS Visits*$ 90/visit – Start of Care
    $ 70/visit – Recertification of Care
    $ 70/visit – Resumption of Care
    $ 70/visit – Evaluation Only (when another discipline has already opened the case)
    $ 65/visit – Follow Up Care
    $ 65/visit – Discharge

    • Mileage and Data reimbursement
