$85.00 per visit on weekdays, $100 per visit on weeknights and weekends


The per-diem RN will be available for home visits to open new cases in Bergen, Passaic, Hudson, and Essex Counties.

We are a non-medical,
private pay personal care agency that provides HHA services to clients at home and in facilities.

This is not a Medicare or Medicaid agency. Nursing Supervisors conduct initial client assessments, develop care plans, and supervise the Home Health Aide. We also provide Skilled Nursing care on a private pay basis.


Minimum Qualifications:


  • Valid RN license from the state they are employed
  • Minimum of 2 years of nursingrelated experience in home health or geriatric care.
  • 5year employment history preferred.
  • Possess adequate professional liability insurance.
  • Valid driver's license.
  • Possession of an automobile with required documentation and valid insurance.

Able to make home visits evenings and weekends, day and night:

  • Possess effective written and verbal communication skills.
  • Be professional in appearance and telephone demeanor

Essential Duties and Responsibilities:


  • Perform client assessments including health status, functional ability, cognitive status, etc.
  • Develop the client's Care Plan.
  • Review and/or revise the Care Plan as needed.
  • Mentor and supervise Certified Home Health Aides.
  • Maintain appropriate client and staff documentation records as per agency policy.
  • Make home visits to assess client status.
  • Keep the Case Manager and Director of Nursing aware of staffing, client, and aide issues
  • Perform skilled nursing visits, as needed (wound care, med box refills, case management)
