    Additional Compensation:

    • Corrections Health Nurses receive an additional 10% hourly premium and hourly differentials for working swing ($2/hr), graveyard ($5/hr), weekends ($3/hr) shifts.
    As a newly hired Advanced Practice Clinician you will receive a:

    * $20,000 retention incentive after (1) one year of continuous regular employment.

    * $20,000 retention incentive after (2) two years of continuous regular employment.

    * $20,000 retention incentive after (3) two years of continuous regular employment.

    All retention incentive bonuses are based on FTE (0.8 FTE and higher will not be prorated) . This retention incentive is taxable, and all regular payroll taxes will be withheld.

    THIS WORK MATTERS


    At Corrections Health, we provide medically needed health care and transitional services to adults and youth in custody at Multnomah County Jails and Juvenile Detention Facility.


    The Advanced Practice Clinician (APC) will work cooperatively with the Corrections Health and the Harm Reduction Program (HRC) teams to provide effective and efficient evaluations, diagnosis, treatment plans and prevention services, for those with substance use disorder or are referred for medication-supported recovery while in custody.


    The Advanced Practice Clinician must consistently demonstrate effective, positive and respectful communication with patients and staff including: active listening, empathy, courtesy, cultural sensitivity, and the ability to promote modification of health behaviors with clients.


    This position will work in both Corrections Health and Harm Reduction Program locations, with service hours and schedules that meet the needs of the population served.

    As an Advanced Practice Clinician in Corrections Health and the Harm Reduction Program (HRC) team , you will help serve in our three Correctional Health facilities and the Harm Reduction Clinic


    • The Multnomah County Detention Center (downtown)
    • Inverness Jail (near PDX airport)
    • The Juvenile Detention Center (NE Portland)
    • Harm Reduction Clinic at Menlo Park Plaza (NE Portland)

    As the Advanced Practice Clinician, other duties will include but are not limited too:

    Clinical Care


    The Advanced Practice Clinician formulates appropriate diagnoses and implements treatment plans based on Harm Reduction Clinic (HRC) practice guidelines, Corrections Health (CH) policies when appropriate, Health Department policies and community standards of practice as well as State and MCHD stage-based behavioral counseling guidelines and training.


    • Provide ultra low barrier medication access to support recovery and reduce risk of overdose.
    • Provide health education to patients.
    • Provide vaccinations as outlined in HRC clinical practice guidelines.
    • Offer contraception services and HIV PrEP based on HRC Program clinical guidelines.
    • Maintain the exam room and orders and review appropriate diagnostic tests and medication.
    • Perform sexual health history, physical examination and specimen collection as outlined in HRC Program and Corrections Health clinical guidelines as appropriate.
    • Performs sexual health screening, genital and gynecological exams, and diagnosis and treatment of genital and sexually transmitted infections (STI).
    • Perform venipuncture as needed.
    • Order and interpret laboratory studies.
    • Prescribe medications such as contraception, HIV Pre-Exposure Prophylaxis (PrEP), doxy-PEP, and antibiotics.
    • Provide follow-up patient consultations, telehealth visits, and other medical conditions.
    • Develop and direct patient-centered care plans with the assistance of other disciplines within the care team.
    • Refer clients to the physician when beyond scope of practice.
    Consultation and Care Coordination


    This position provides consultation to nursing staff, Harm Reduction Clinic (HRC) program staff, Health Department Clinics, and community providers as indicated for continuity of client care.

    The Advanced Practice Clinician assesses and counsels patients regarding additional care needs, including preventative healthcare, specialty or rehabilitative services, or high risk / acute care.

    Providing appropriate advice on the management of SUD to county or community health care providers, following HRC and STI Program clinical guidelines and in accordance with Centers for Disease Control and Prevention (CDC) treatment guidelines.


    • Provide in-service to staff as a group or individually, as requested.
    • Partner with the Senior Harm Reduction Specialist and Corrections Health to enhance and improve MSR across relevant sites
    • Participate in precepting students, volunteers, and new staff when appropriate
    • Make appropriate referrals to care in coordination with the patient and care team.
    • Establish need for specialty and rehabilitative services and make appropriate referrals.
    • Assist in developing and implementing protocols, procedures and other teaching tools for employees.
    • Provide direction to Community Health Workers, Community Health Nurse and Clinical Service Specialists to implement aspects of care plans within their scope and training.
    Documentation and Reporting


    The Advanced Practice Clinician is responsible for the timely and accurate documentation of client care through the use of electronic health record charting.


    • Review client records for needs for medications, treatments, education or referrals.
    • Document appropriately and clearly in the electronic health record (EHR) system.
    • Utilize panel based information technology tools to prioritize and direct proactive care for patients assigned to the provider panel.
    • Apply division, department, and all healthcare requirements for HIPAA and other compliance standards.
    • Partner with department and evaluation staff to identify priority MSR outcomes and workflows for evaluation and improvement.

    Minimum Qualifications/Transferable Skills*:

    • Master's Degree in Nursing AND one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.

    • Experience working in a correctional facility.
    • Training or experience in working with Electronic Health Record systems.
    • Experience working with targeted groups and with ethnically and culturally diverse populations.
    • Experience in medication assisted recovery (e.g. medications for opioid use disorder, MOUD or MAT).
    • Experience in opioid withdrawal management.
    • Demonstrated understanding, implementation, and promotion of harm reduction principles in all aspects of clinical care.
    • Transferable skills: Your transferable skills are any skills you have gained through education, work experience (including the military) or life experience that are relevant for this position. Be sure to describe any transferable skills on your application and clearly explain how they apply to this position.
