    Multnomah County Community Health Center, provides quality health services for people who experience barriers to accessing care.

    The vision of the health center is to provide services that are integrated and connected to support optional health, reducing wait times and optimizing access.

    The Primary Care Division assesses need and develops services that promote 100% access and zero disparities for the screening and treatment of illness.

    A primary focus of services is on prevention and client education to promote healthy choices. Clients are seen in geographically dispersed sites throughout the county.

    The APC Fellow works as part of the primary care team in a supportive learning environment.

    With guidance, the fellow provides clinical care, sets treatment plans, provides education and resources within the Health Center and external programs that best meet the client's needs.

    The position determines diagnosis for condition and treatment, and sets high quality medical care to improve and maintain health status for a diverse client population within a busy, team-based setting with an emphasis on prevention of disease and promotion of health.

    The position also establishes a therapeutic relationship with a diversity of clients and families, including refugees and low-income population groups.


    As the APC Fellow, you will:

    • Learn and gain experience on how to successfully practice in an FQHC setting. This is a supported year long fellowship that builds towards independent practice management of a client panel.
    • Gain experience in understanding all aspects of primary care clinical and population health management, including
    • Preventative Care utilizing a team based approach
    • Providing care in a dynamic model of care that accommodates clients needs within various settings including virtual care, telephone triage and assessment, walk in/urgent care assessment
    • Panel management managing the indirect clinical care needs of a client panel
    • Diagnose and treat acute and chronic medical conditions and diseases according to scope of practice to include conducting a medical history, conducting physical exams, perform treatments such as suturing simple lacerations and wound care, etc., ordering and interpreting diagnostic tests, devising a course of treatment, discussing alternative treatment, risk and benefits of treatment, prescribing and monitoring medications, appropriate counseling of patient and reassessment of the patient at appropriate intervals.
    • Provide patient education on disease treatment options, nutrition, exercise, and health maintenance behavior according to the treatment plan.
    • Utilize a team to prioritize and direct care around annual comprehensive health screenings, preventive health practices, chronic disease management, community resources and mental health needs of clients.
    • Utilize panel based information technology tools to prioritize and direct proactive care for patients assigned to the provider panel, including preventive health screening and chronic disease care management.
    • Provide followup on utilization issues (ED, hospital admissions). Work within teambased care to best provide quality medical determinations.
    • Establish need for specialty and rehabilitative services and make appropriate referrals.
    • Assess and counsel patients regarding preventive care needs; delivers services or make appropriate referrals. Determine need/schedule for high risk referrals.
    • Apply knowledge of internal and external resources and programs to best support patient care and health maintenance or improvement.
    • Seek clinical consultation from senior clinicians as needed as part of practice management.
    • Provide consultation to Nurses and support staff. Prioritize and direct activities of the care team.
    • Coordinate with Behavioral Health providers to effectively integrate care into care plans for the client.
    • Complete patient documentation and manage electronic inbasket according to established policies and procedures.
    • Maintain high quality notes and recommendations for treatment and next steps in care plan as part of a teambased approach.

    Minimum Qualifications/Transferable Skills*:
    Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.


    Education/Training/Experience:

    • Master's Degree AND
    • License to practice as a Nurse Practitioner or Physician Assistant as outlined by[Oregon Revised Statutes (ORS ])
    • CPR/BLS certification
    • Within the first 12 months of Nurse Practitioner or Physician Assistant Graduation

    • Demonstrated knowledge of methods and techniques of diagnosing and treating a variety of diseases and injuries;
    • Ability to effectively screen and gather required information in a professional, culturally competent and customer service oriented manner;
    • Ability to motivate clients through discussion and persuasion;
    • Ability to communicate clearly and concisely, both orally and in writing;
    • Ability to establish and maintain cooperative working relationships with small teams and others contacted in the course of work; including school administrations, school districts, community partners, and Multnomah Education Services District
    • Ability to be flexible and manage multiple priorities
    • Transferable skills: Your transferable skills are any skills you have gained through education, work experience (including the military) or life experiences that are relevant for this position.
