    The Primary Care Physician (PCP) at ChenMed demonstrates:



    • Accountability for outcomes: The PCP takes responsibility for outcomes, providing high-quality clinical care and cost-effectiveness for a panel of up to 450 patients. They understand the impact of building trusting relationships and aiding patients in behavior change on patient outcomes.
    • Coaching for health: Acting as a health coach, the PCP assists patients in setting and achieving short and long-term health goals, collaborating with patients to work towards these goals, and regularly monitoring progress towards improved health.
    • Simplifying for action: The PCP simplifies and prioritizes effectively to make behavior change actionable for patients in reaching their health goals and when guiding their care teams towards performance targets.

    We emphasize outcomes and value-based care. PCPs' success and progression to partnership status are measured against metrics for their patient panel, such as patient admissions per thousand, appointment slot utilization, CGCAHPS scores, clinical gap closures, and medical cost measures. PCPs are assigned goals for these metrics and are expected to work towards them in collaboration with center and market leadership and their care teams. Creating a positive culture of love, accountability, and passion is crucial in our medical centers, and as leaders, PCPs are responsible for championing this culture alongside center leadership.


    The PCP should demonstrate the ability to work independently and collaboratively with other healthcare professionals. They will closely collaborate with managers and medical directors to ensure compliance with guidelines, participate in risk and quality management programs, and attend relevant meetings to advance patient health and company objectives. PCPs are expected to meet departmental goals/objectives, performance standards, regulatory requirements, quality patient care standards, and operational policies.


    ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

    • The PCP provides independent care for patients with acute and chronic conditions typically seen in elderly patients.
    • The PCP assumes full responsibility for patient care, engages specialists as needed, and remains involved in the detailed care of the patient.
    • The PCP communicates with hospitalists whenever their patients are hospitalized.
    • The PCP is accountable for assessment, diagnosis, treatment, management, education, and care coordination for patients with acute and complex chronic health needs.
    • The PCP leads a care team comprising a care promoter, care facilitator, and care coordinator for office-based patients.
    • For patients unable to visit the office, the PCP collaborates with the transitional care team and other resources depending on the market.
    • The PCP plays an active role in center management, covering for absent providers and participating in patient and provider recruitment.
    • Other duties as assigned by management.

    • Quality: Strive for high-quality care, collaborate with care teams for quality outcomes, and seek continuous improvement in practice and center operations.
    • Influence: Influence patients, teams, and oneself for optimal outcomes.
    • Self-Care: Prioritize personal wellness to deliver the best care to patients.
    • Proficient in Microsoft Office Suite and other relevant software.
    • Willingness to travel locally and occasionally regionally and nationally as needed.
    • Fluency in English, additional languages may be required based on center needs.
    • Exercise of independent judgment is necessary.

    EDUCATION AND EXPERIENCE CRITERIA:

    • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related field required.
    • Active State Medical License or ability to obtain one.
    • Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics preferred.
    • Maintain specialty board certification and DEA number.
    • Basic Life Support (BLS) certification required within the first 90 days of employment.
