The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. They are strongly focused on improving their members' physical, mental, and social wellbeing through personalized clinical care and therapies. They are currently hiring due to growth.

The provider is responsible for managing a panel of long-term SNF residents, focusing on treating patients in place in collaboration with a primary care team.

The provider conducts comprehensive assessments, develops and delivers individualized plans of care, attends care team meetings, communicates with family members / caregivers, and ensures that the care team understands the patients goals and needs of care.

assess and plan healthcare that is individualized to the patient
- review and interpret medical, surgical, and social histories
- interpret laboratory and other diagnostic data and recommend treatments / actions
- perform direct care management support
- educate care teams on patient needs and early identification of changes in condition
- work collaboratively with physicians, care coordinators, facility staff, family members, etc.
with members of the care team.
some previous experience in SNFs, LTC facilities, etc.
