***: The Community Specialist shares responsibilities with the Information and Referral Supervisor for WMEC's activities related to the maintenance and dissemination of information regarding services and programs available to consumers. She/he holds a central position in directing intakes, assessments, counseling and referral of consumers and their caregivers who require information and/or services. The bilingual/bicultural Community Resource Specialist engages in activities designed toward eliminating barriers and improving the conditions and well-being of Hispanic clients.


ESSENTIAL DUTIES AND RESPONSIBILITIES:


  • Be knowledgeable of and comply with all state, federal and agency regulations, policies and procedures pertaining to elder services and WestMass ElderCare's mission.
  • Develop and maintain cooperative relationships with institutional and community service organizations in order to effectively utilize all available resources for elders.
  • Assists WMEC staff that needs resources and/or is working in Information and Referral.
  • Assists in providing information and service to Spanish speaking callers.
  • Initially assess calls for service per established standards, evaluating problems, resources and needs; and provides information and counseling regarding options for services and/or resources.
  • Works with underserved populations. Participates in outreach to underserved populations.
  • Coordinates resource data bank information, monitoring the accuracy and currency of the information.
  • In crisis situations is able to contact and coordinate with appropriate emergency response network.
  • Maintains accurate documentation of calls, referrals, and follow-up in compliance with confidentiality requirements.
  • Provides information and training to agency staff regarding Community Resources and programs as directed by the Community Resource Supervisor.
  • Submits timely reports and statistics as requested by the Supervisor, Assistant Director, or another member of Management.
  • Participates in on-site and state wide training pertaining to Community Resources.
  • Provides verbal and written translation as requested by Supervisor or member of the Management Team.

OTHER DUTIES:


  • Performs other related duties within the scope of the responsibilities of this position as requested by the Information & Referral Supervisor.

REQUIRED KNOWLEDGE SKILLS AND ABILITIES:


  • Possesses the ability and willingness to support the mission statement, goals and policies of WestMass ElderCare; and has the ability to interpret, understand and adhere to the regulations, standards, and procedures as set forth by the Agency and the Executive Office of Elder Affairs.
  • Bachelor's Degree in a relevant field is required.
  • Demonstrated experience and/or strong interest in the field of human services via previous employment, internship, volunteer activity and/or additional studies preferred.
  • Bilingual Spanish REQUIRED.
  • Possesses the ability to communicate orally and in writing effectively.
  • CIRS-A certification or the ability to become certified.
  • Possesses knowledge of the aging process, community resources and social health intervention techniques desirable.
  • Possesses knowledge of or the ability to learn federal and state laws and regulations in regards to elder services.
  • Knows how to look for and access resources.
  • Possesses interviewing, assessment, and documentation skills.
  • Possesses the ability to effectively communicate with clients, family members, service providers, and coworkers.
  • Has the ability to initiate and sustain appropriate interpersonal relationships.
  • Possesses the ability to express ideas concisely and clearly, both orally and in writing.
  • Possesses demonstrated organizational and time management skills.
  • Has the ability to manage multiple projects and assignments and prioritizes work to stay in compliance with regulations, policies, procedures and time lines.
  • Must possess a working knowledge of Microsoft Office Programs and have the ability to learn other applicable programs.

CERTIFICATIONS, LICENSES, REGISTRATIONS:


  • CIRS-A certification preferred.
  • Satisfactory background check that includes CORI, SORI, SAMs GSA, Mass Health Exclusion List and OIG check.
  • Valid driver's license required. Vehicle available during all working hours. Vehicle should be Registered and inspected in accordance w 540 CMR 4.00.

Pay:
$ $22.00 per hour
