HOME CARE CASE / EVV COORDINATOR


Fast paced Home Health Care Company in Philadelphia is in search of an enthusiastic, multi-tasking individual with excellent communication skills to join our growing team If you are motivated, detail oriented, professional and ambitious we are interested in speaking with you about staffing, scheduling and/or patient care coordination.

As a Home Care Case Coordinator you will be responsible for scheduling field staff, managing phone calls, and responding to client inquiries as well as addressing client needs.

This employment does include some off-hours / weekend responsibilities.


Responsibilities:


  • Utilize our webbased care management system (Electronic Visit Verification) to monitor caregivers for ontime arrival, manage clockin/clockout system, monitor care logs, and update client and caregiver records.
  • Work closely with our clients to schedule caregivers for home care and problems solve issues that arise.
  • Accurately staff clients with caregivers based on location, time, skills/needs, and requests.
  • Effectively and accurately communicate with caregivers, family members, clients, case managers, and office staff.
  • Perform inhome assessment visits and calls as required.
  • Complete incident reporting as necessary.
  • Contact healthcare providers to obtain documentation, authorizations and information as needed
  • Ensure all state and federal rules and regulations are strictly adhered to, including each specific MCO's requirements
Scheduling agency home health care aides accordingly and ensuring agency aides work their scheduled shift.

Managing last minute calls outs, finding appropriate coverage.

Developing an on-call pool of agency aides with various shifts in coordination with Human Resources

Communicate with referral sources and Case Managers to provide an excellent customer service experience

Listening to needs of clients and matching them to the appropriate caregiver(s)

Manage Caseload.

Review the care plan with patients and caregiver and support coordinator of MCO's.


Pay:
$ $22.00 per hour
