The Phoenix Center for Rehabilitation & Pediatrics is a 200+ bed skilled nursing facility caring for both the geriatrics and pediatrics populations.

We are currently seeking a Wound Care Registered Nurse to join our team.

Responsibilities as a Wound Nurse:

The primary purpose of this position is to be responsible for coordinating wound care assessments, prevention, and to provide recommendations to the medical staff


Other responsibilities include:

  • Develop nursing care plans for residents at risk or currently with skin integrity issues including treatments protocols
  • Monitor and assess any new skin observation alerts
  • Work in conjunction with the DON in supervising the wound care programs
  • Provide basic wound care training to all new nurses and therapy staff, provide ongoing inservices and training relating to all skin and wound care
  • Evaluate, plan, and implement effective plans of care with open communication with both residents and families
  • Make sound decisions in response to changes in condition and/or response to treatment
  • Educate staff on protocols that preserve integrity of the skin and promote wound healing

Requirements for a Wound Nurse:


  • Current license as a Registered Nurse in the State of New Jersey
  • Experience as Wound Care Nurse in a longterm care setting a plus
  • Must have excellent verbal and written communication skills
