Job Summary:


We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health and Hospice team on a full-time or part-time basis.

As a Home Health and Hospice RN, you will provide high-quality care to patients in their homes, focusing on patient-centered care and comfort.


Responsibilities:


  • Conduct a minimum of 2025 visits per week in Palm Springs and surrounding areas
  • Perform assessments, develop care plans, and implement care interventions for home health and hospice patients
  • Administer medications, IV treatments, and other medical interventions as prescribed
  • Provide emotional support and education to patients and their families
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Document patient information accurately and thoroughly

Benefits:


  • Competitive per diem rate ($110 for home health case starts, $135 for IVs, and $300 for opening hospice cases)
  • Travel included in per diem rate
  • Flexibility to work 18 visits per day, depending on the visit

Requirements:


  • Current California RN license
  • Home Health and Hospice experience preferred
  • Strong communication and interpersonal skills
  • Ability to work independently and manage time effectively
  • Reliable transportation (company car provided)
  • Basic computer skills
