Personalized Nursing LIGHT House is a premier behavioral health care provider with 34 years of specialized addiction treatment service serving the adult population. We have locations in Plymouth, Canton, Brighton, Dearborn Heights, & Madison Heights. Specifically, our intent is to increase client well-being, decrease addiction severity, improve the quality of clients' mental and physical heath, and contribute positively to society. All services will integrate substance use disorder, mental health, and primary health care treatment in cohesive and client-centered programs.
POSITION SUMMARY
We are seeking a certified Peer Recovery Coach to serve as our Extended Care Coordinator (ECC). The Extended Care Program is a longitudinal recovery support program geared at providing clients connection and support as they move through their first year of treatment and recovery. The clients who participate are given regular home calls, resources and supports to help them overcome risks of relapsing into active addiction and help them to both heal and to build a foundation for lasting sobriety in the community.
As a Care Coordinator you will be assigned a caseload of clients to work with through a planned 12-month period of their early recovery from addiction. Your responsibilities include regular meetings with the clients, phone calls to clients for monitoring and support, guiding clients with treatment options and monitoring clients for adherence to treatment recommendations, assigning dates for client to provide a UDS, and sharing information with clients and their support network to foster recovery and tracking recovery progress. The ECC will be responsible for completing various forms, data collection, reporting to their department supervisor and participating in the production of necessary reports.
Knowledge of recovery resources and supports, a willingness to learn new skills and, above all, to provide a kind, caring and compassionate approach to client care is necessary. Certification and experience as a Recovery Coaching is required. In addition, you will provide Recovery Coach support to PNLH's clients currently enrolled in intensive levels of care.

REQUIREMENTS
PNLH preference for candidates with training in, and experience with, providing trauma related services. We take affirmative efforts to hire individuals with disabilities, including those in recovery.
