Healthy Behavioral Support Services Coordinator-Combined Position for the Indiana WISEWOMAN program and the Diabetes Resource Center at United Health Services; both concentrate on prevention, access and disease management
Classification: Non-Exempt, 37.5 Hour Full Time Position.
Hours and Location: Monday through Friday at the home office and some Saturdays will be required. Reporting to a regional coordinator & the executive director
Start Date: March 1, 2026 or later
Healthy Behavior Support Services Coordinator– hours a week
Diabetes Client Services – 5-7.5 hours a week (13.3%-20%)

Minimum Qualifications, Skills, Abilities & Background:
- Education:Associates degree or higher
- A concentration in nutrition, clinical health or social work highly desired
- Ability to communicate effectively, both orally and in writing, in
- Knowledge of the organization of public health services, food assistance and nutritional and/or physical education resources is a plus
- Ability to collect, track & organize data and technical information
- Organize and prioritize multiple tasks at a time
- Willingness to learn & use objectives in goal planning, motivational interviewing and developing broad range of education in nutrition & physical activity required
- Desire to help your community and improve the health of others
- Relate comfortably, sensitively, and respectfully to consumers
- Maintain client, patient & program confidentiality (HIPAA)
- Must have reliable transportation and a valid driver's license
- Utilize Microsoft Office Products
- Be comfortable with data tracking tools and online databases
Job Responsibilities:
Goal #1: To enhance the prevention availability and ease for the participants IN WISEWOMAN Ensure participants in state programs are referred to appropriate community programs for Healthy Behavior Support Services (HBSS) and lifestyle behavior change efforts
· Find, outreach and enroll participants into the Indiana WISEWOMAN program, includes scheduling appointments/labs in house
· Work with the Indiana Department of Health and WISEWOMAN enrollment sites to receive participant information and data to UHS
· Take small blood samples, and do labs on a Cholestech machine
· Help ensure that UHS follows all rules for safe needle and hazard materials including PPE
· Outreach to programs which have approved Healthy Behavior Support Services already in their facilities for partnerships. Work with the coordinator to create agreements with these facilities.
· Maintain current Memorandum of Understanding (MOUs) for UHS with partnering agencies
· Develop and/or Implement Bidirectional Referral System with partners alongside the regional coordinator and health coaches
· Become a health coach by participating in the IN WISEWOMAN trainings set up by the IDOH.
· Follow the guidance & rules of the IDOH providing health information unless licensed to do more. If that is the case, keep UHS and IDOH current on licensing and any changes.
· Become certified in the Blood Pressure Self- Monitoring Program
· Work with other UHS staff to ensure data, including referrals, sessions completed and overall program completion is submitted to IDOH timely
· Help to create, plan distribution of participants' materials needed to complete programs, and oversee the process.
· Attend & participate in monthly & quarterly WISEWOMAN
Goal #2: Attend to people with diabetes in crisis by supplying immediate need coverage and case management to lift them to stability in their healthcare needs.
· Ensure clients and referrals of the Diabetes Resource Center are served in a timely, professional manner
· Work to develop and manage the Diabetes Resource Center programs to ensure the attainment of organizational goals and work plan objectives
· Maintain databases and other evaluation tools to track and assess activities
· Ensure diabetes grant requirements are tracked and met by respective deadlines to ensure success of the organization and programs.
· Communicates issues regarding programs, clients, etc. to the director in a timely manner
· Develops outreach materials for programs and events, as directed
· Assist with fundraising efforts including but not limited to the Bad Pants Open golf outing
· Track, sort & thank monetary and in-kind donations
· Learn and know resources available to people in our community including access to healthcare & basic public insurance products such as HIP, Medicaid, Marketplace & Medicare
· Refer clients to the proper resources to help them reach stability in their access to diabetes care
· Represent the Diabetes Resource Center and UHS and community health events & meetings

Pay is hourly and will vary depending on experience. Rate may be increased after certifications are completed.
