COMMUNITY SUPPORT SERVICES COORDINATOR

Responsibilities


Under the supervision of the Director of Residential Services or other Executive, the Community Support Services Coordinator is responsible to provide community mental health rehabilitative support services to consumers living independently in the community, and provide clinical and/or administrative supervision to program staff.

The CSS Coordinator meets face to face with consumer to perform comprehensive rehabilitation needs assessments and completes consumer rehabilitation plans and ensures time completion of encounter notes and other chart material.

In addition, as an employee of SJBHR, the Community Support Services Coordinator will assume an active role in improving his/her program outcomes and overall organizational performance through participation in agency quality improvement activities and other management meetings as necessary and appropriate.


Education and Experience:

LCSW, LPC, or RN must be licensed by the applicable New Jersey State professional board.

Valid driver's license is required. Driver's abstract must meet insurance carrier's requirements. Upon hire, must satisfactorily complete the SJBHR orientation and competency based adult mental health rehabilitation program training.


Graduation from an accredited college/university with a Master's Degree in social work or other related behavioral health, or, graduation from an accredited professional nursing program.

Two years of community mental health experience is required.
