- Conduct comprehensive assessments to identify patient needs and craft individualized plans of care.
- Coordinate care with physicians, social workers, therapists, and other healthcare providers.
- Educate patients and families on conditions, treatments, and available resources.
- Advocate for patient rights and access to necessary care.
- Monitor patient progress, adjust care plans as needed, and evaluate outcomes.
- Ensure efficient discharges and transitions to other care levels.
- Maintain detailed documentation of patient interactions and care plans.
- Collaborate with case management team to optimize resource use and patient outcomes.
- Active, unencumbered RN license in California.
- At least 2 years of experience in Case Management, preferably in an acute care setting.
- Supervisory experience in an acute healthcare facility.
- In-depth knowledge of healthcare systems, reimbursement, and community resources.
- Exceptional communication, collaboration, and interpersonal skills.
- Strong organizational and time management abilities.
