- Conduct comprehensive assessments of patients with diabetes, including medical history review, glucose monitoring, and lifestyle evaluation.
- Develop personalized diabetes management plans in collaboration with patients, incorporating nutrition, exercise, medication, and self-care strategies.
- Provide individual and group education sessions on diabetes self-management, insulin administration, blood glucose monitoring, and complication prevention.
- Support patients in setting and achieving glycemic targets, weight management goals, and overall wellness objectives.
- Collaborate with healthcare providers, including physicians, nurses, and dietitians, to coordinate care and optimize patient outcomes.
- Stay updated on the latest research, guidelines, and technologies related to diabetes management, and integrate best practices into patient education and care.
- Certified Diabetes Educator (CDE) certification required.
- Prior work experience as a Certified Diabetes Educator or in a related role preferred.
- Expertise in diabetes self-management education, insulin therapy, carbohydrate counting, and blood glucose monitoring.
- Strong communication, teaching, and motivational skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
