Pinnacle Home Care, LLC. is currently seeking an experienced
Physical Therapy Assistant (PTA). We provide one-on-one skilled physical therapy to patients living in their homes. PTA provides rehabilitation and implements a plan of treatment in conjunction with the physician and the patient.

PTA collaborates with Physical Therapist as well as other interdisciplinary team members regarding the care and treatment necessary to improve the patient's health condition.

Responsibilities:

• Make home visits to patients.
• Implements patient's individualized treatment plan as established by Physical Therapist.
• Work with other members of the rehabilitation team to implement programs consistent with the needs and capabilities of each patient
• Educate and instruct patients on activities necessary to promote their health, safety, and independent living.
• Accurately and timely document observations, interventions, and evaluations utilizing a touch pad tablet that is provided to you by the company.

Qualifications:

• Current license to practice as a Physical Therapy Assistant in RI.
• Valid CPR license.
• Valid driver's license and proof of insurance coverage.
• Ability and willingness to work in a team environment.
• Ability to travel between patient assignments within a 25mile radius.
• Bilingual is a huge plus

Pay:
$32.00 per hour


Expected hours:
No more than 24 per week
