- Participates in selecting strategies for evidence-based wellness/prevention initiatives
- Develops initial care plans with specific goals for enrollees
- Communicates with medical/subspecialty providers for care coordination
- Provides health education, treatment recommendations, and monitors assessments
- Facilitates use of EHR and HIT for service linkage and communication
- Reports performance measures and meets regularly with the care team
- Assesses patients for medication suitability
- Performs medical/mental health evaluations and lab test reviews
- Conducts patient evaluations and reviews medications regularly
- Discusses drug interactions and ensures proper tapering/discontinuation
- Must be licensed in State of Michigan
- Experienced in chemically dependent and mental health patient treatment
- Minimum 1 year experience preferred
- Participate in ongoing training for quality medical care
