    Responsible for the coordination of the care and management of the transplant recipient. Must be able to function with considerable independence in the management of the transplant patient.     Responsibilities:

    Leads performance improvement (Pl) initiatives consistent with organizational and service line priorities and goals
    Coordinates clinical collaborative efforts to drive change in clinical practice and outcomes
    Participates in implementation of patient safety initiatives and promote National Patient Safety Goals either separately or concurrently with other projects
    Partners with Risk Management to perform failure mode effects analyses in identifying high risk processes and systems to proactively promote safe care
    Integrates knowledge of external standards, including state and federal regulations pertaining to quality and safety
    Researches literature or acceptable standards and guidelines to assist in development of practice guidelines (standing orders, policy/procedures, clinical pathways, protocols, etc)
    Monitors quality performance, keenly identifying variation in the use of evidence based practice and to determine the need for peer review
    Implements EBP, tools, tactics and techniques to improve clinical, operational and service excellence and help share best practices
    Supports systems and standardization, while recognizing the individuality of each service line
    Conducts medical record mortality reviews using Institutes for Healthcare Improvement (IHI) based criteria; looking for common causes and trends
    Conducts medical record reviews and interventions for concurrent and retrospective hospital acquired conditions (HAC) and patient safety indicators (PSI)
    Partners with Medical Staff Office in process of medical staff quality management programs, including peer review & Ongoing Professional Practice Evaluation (OPPE) and Focused Professional Practice Review (FPPE) quality information
    Performs concurrent or retrospective medical record review for quality concerns; establishes timelines and exercises discretion and judgment when identifying concerns for review to improve processes
    Supports several service line medical staff departments in performance improvement initiatives
    Recommends areas for quality review; assist to identify high volume, high risk procedures across hospital and outpatient areas
    Formulates appropriate review criteria and presents for physician review and approval
    Prepares data reports for medical staff quality initiatives utilizing current concepts in quality improvement methodologies
    Assists in integrating data abstraction, quality monitoring, and reporting activities which support medical staff quality improvement activities compliance with service line required accrediting bodies
    Assists physicians with quality assessment reviews, prepare data and present reports to medical staff; collects data through chart reviews, performs data analysis, and presents information in an effective format to designated staff
    Evaluates potential standards of care that may need to be reviewed by medical staff including patient/ family grievances, team complaints, inpatient mortality, surgical/ procedural complications, clinic operations, and blood utilization
    Monitors reports for patterns and trends and act upon these appropriately
    Provides service line-specific studies that require concurrent review, collection of data, and assistance in analyzing and sharing data
    Conducts quality-related audits for select third party vendors providers to facilitate ongoing education and improvement to assigned populations
    Applies quality improvement study design and implementation including selection of valid and reliable indicators, coordination of monitoring and evaluation activities, and medical record reviews
    Analyzes data and prepare concise, accurate and meaningful quality management reports in accordance with quality management principles
    Evaluates compliance with core measures including retrospective chart abstraction, outlier identification, dissemination of results and assistance in accompanying action plan
    Partners with clinical informatics in providing consultation on core measures, quality and safety measures related to efficient and effective retrieval of data within the electronic health records
    Works in parallel with clinical Informatics to ensure that clinical care-related changes are also carried over into the Electronic EMR environment
    Compiles QA data, prepares reports, and gives statistical analysis of QA information
    Assists Nurse Managers to lead monthly Program Quality Management meeting
    Develops standard operating procedures for Program manual, data management and quality assurance, monitoring and storage of data; ensures timely review of new/revised procedures

    Qualifications:

    Required Qualifications

    • Bachelors degree in nursing
    • Minimum of three (3) years relevant work experience in a healthcare setting

    Preferred Qualifications

    • Masters degree
    • Clinical knowledge and expertise in a clinical service line specialty
    • Nationally recognized quality-related certification
    • Experience in quality improvement principles, performance improvement methodology, project/data management and research
