- Focus on the clinicalaspects of patient care by helping patients in understanding theirdiagnosis, treatment options and the resourcesavailable.
- Help patients navigate their way totimely and quality care from suspicious findings to treatments,while addressing tangible barriers to patientcare.
- Serve as an essential link betweenpatients and all other care providers and follow patient throughcontinuum of care.
- Facilitate appointments forconsults and support services within established standards.
- Function as a member of an interdisciplinaryteam to assess, secure and coordinate appropriate services.
- Secure and execute financial grants from stateand or national programs if applicable and assurecompliance.
- Work with Cancer Registry staffand others to collect data, track outcomes and support strategicwork plans.
- Develop educational tools forpatients and providers as needed.
- Responsiblefor educational outreach efforts to community and to key customers(physicians, office staff, diagnostic staff, etc.) to establish andmaintain positive working relationships.
- Perform all other duties asassigned.
- Education / Professional Qualifications: HighSchool diploma or equivalent required. Bachelor's degree inNursing
- Requires problem solving, decisionmaking and critical thinking skills
- Requiresexcellent leadership, organizational, written and verbalcommunication and interpersonal skills.
- Computer literate with software office skills plus basic computerknowledge.
- More than 3 years ofnursing experience.
- Experience inoncology or case management or nursing specialty is aplus.
- Active State Licensed BSNrequired.
