Job Profile

Key responsibilities:


  • Conduct Mosaic patient onboarding interviews, by phone and/or videoconference:
  • Educate patients about our pharmacy service;
  • Interview patients about their medications, providers, and health conditions;
  • Thoroughly and accurately document patient information in our pharmacy management system; and
  • Synchronize medication regimens and schedule initial medication delivery
  • Serve as pointofcontact for new patients until delivering first medication delivery
  • Promptly followup with patients to resolve outstanding questions
  • Complete delegated pharmacy technician acts as instructed by a supervising pharmacist, or other duties as assigned by management
  • Collaborate effectively with supporting pharmacy technicians to ensure accurate data entry and successful prescription transfers, working with prescribers to obtain new prescriptions, and completing telephonic medication review calls with patients.
  • Ensure the onboarding process meets company standards and patient expectations
  • Advocate for continuous improvements in the onboarding process

Qualifications

  • Proof of an active Virginia pharmacy technician license required
  • High School diploma or GED equivalent is required
  • Minimum of 24 years of experience in direct patient communications in settings such as home health, hospital, primary or specialty physician practice, pharmacy or similar. Experience collecting medication histories is a strong plus.
  • Ability to ask relevant openended questions and accurately transcribe responses.
  • Ability to work in a fastpaced work environment where positive change is the norm, and managing priorities will be needed. Careful listener with strong attention to detail, able to identify potential concerns, ask intuitive questions, and resolve issues through your own effort or escalation to a supervisor.
  • Proficient Spanish speaking capabilities, in addition to fluent English, highly desirable.
