Job Summary:

This position is currently under evaluation and needs to abide by the guidelines set out in the relevant collective bargaining agreements and/or NLRB definition of a confidential employee. Prior to being posted, HR Consulting must review this position.

Responsible for identifying, monitoring, and reaching out to patients to achieve the goals of outreach programs and enhance the quality of care. Keep track of records, statistics, and databases related to local, regional program objectives, and regulatory mandates.

Collaborate with other healthcare professionals to help patients discover and utilize resources that enhance the quality of care.

Essential Responsibilities:
  • Under direct supervision, monitor patient populations to gather statistical data pertinent to specific healthcare management goals and ensure that patient records are current.
  • Collect healthcare information from patients, maintain care management databases, and produce reports for specific care management programs.
  • Apply established measures and criteria to identify patients eligible for specific care programs.
  • Develop letters, forms, and other materials required to educate and gather information from patients involved in particular care programs, under direct supervision and using established formats.
  • Inform enrolled members in care management and health education classes and monitor their participation.
  • Disseminate educational resources.
  • Reach out to patients via phone and correspondence to ensure compliance with medication use, physician visits, and self-care management skills.
  • Assist Physicians by preparing annual screening, utilization, compliance, and other reports related to care management and program efficacy.
  • Utilize databases and reports under supervision to verify patient adherence to specific care programs and regulatory standards. Act as a link between the patient community and other Kaiser resources.
  • Provide recommendations for patient referrals as needed due to noncompliance with treatment plans or to meet program or regulatory requirements.
  • Perform other assigned duties as necessary.

Additional Requirements:
  • Strong Customer Service skills and proficient verbal and written communication abilities to independently craft correspondence.
  • Capable of working in a Labor/Management Partnership setting.
Preferred Qualifications:
  • Minimum of three (3) years of progressively responsible experience in healthcare.
  • Preferably a Bachelor's degree in Health Care (LVN or related field of study).
Notes:
  • May participate in KP Membership events, should be able to lift up to 35 pounds. Occasional work on weekends included.
  • Temporary role lasting around 5 months.
