Description
Hiring for full time RN case managers to service patients in our Sarasota ALF.

The vast majority of our visits are in assisted living facilities, which greatly reduces drive time comparted to other home health agencies.

Duties include case management of multiple patients, OASIS completion, and general skills (injections, wound care etc).

You will work closely with the administrative staff of the building and Home health DON to ensure the patient has the care that is needed, as well to identify any residents in the buildings that may need home health services.


Job Types:
Full-time, Per diem


Pay:
$75, $80,000.00 per year
