    Job Description:

    Join one of the nations most comprehensive academic medical centers, University of Chicago Medicine, in a dynamic role as the Patient Care Coordinator for our Hematology/Oncology clinic. Our collaborative teams of cancer experts offer medical, surgical and radiation therapy options not widely available, including breakthrough immunotherapies and new surgical approaches that minimize scarring and reduce recovery times.

    In this role, the Patient Care Coordinator isresponsible for guiding patients through the health care system and ensuring those patients and their caregivers are able to access culturally and linguistically appropriate services in a timely and cost-effective manner. The Patient Care Coordinator coordinates appointments for new patients with multiple providers and ancillary appointments before and after their first visit.

    Responsibilities include:

    • After initial appointment is scheduled serves as a point of contact with patients and is responsible to schedule appointments, including surgery, and coordinate referrals for testing, imaging, procedures, social work, physical therapy, occupational therapy, labs, and other care and support services at the hospital or in the community; contacts patients via phone, email, MyChart, and/or letter to schedule, confirm, and send out reminders regarding appointments.
    • Follows up with patients at regular intervals as defined to ensure that patients arrive to scheduled appointments on time and prepared. Fields patient and caregiver calls and questions related to appointments, ancillary services, or other matters related to coordination of care. Supports triage of In-Basket messages, and communicates effectively with hospital staff, clinicians and physicians involved in the care of the patient.
    • Obtains prior authorizations, medical records, slides, specimens, faxes, and other items necessary for coordination of care; follows up with outside facilities/labs and other ancillary services for testing (including imaging) results and medical records and shares results with appropriate services as needed, including submitting results to Medical Records and patients community physician(s).
    • Initiates and prepares FMLA/Disability paperwork with patient, completes appropriate forms as directed by clinic staff, including Nurse Associates and Physicians; Documents in the electronic medical record, including information provided to patients and their caregivers about appointments and referrals; Creates EPIC notes for supporting documentation
    • Performs other related duties as assigned.

    Required Qualifications:

    • High School Diploma or equivalent from accredited institution
    • Minimum of two years experience working in a high-volume healthcare environment
    • Excellent telephone and personal communication skills are required as is proficiency with computer systems and programs
    • Ability to multi-task and prioritize is crucial to success in this position
    • Compassionate listening skills are required for patient and family distress related to new diagnosis
    • Ideal candidate has strong interest and desire for extensive patient and family interaction
    • Requires experience with and high commitment to customer service

    Position Details:

    • Job Type/FTE: Full Time (1.0 FTE)
    • Shift: Days 8-Hour Shift
    • Unit/Department: Hematology/Oncology
    • CBA Code: Non-Union

    In this role, we anticipate that you will generally work approximately 40 hours per week. We anticipate that you will generally be scheduled to work the first shift, from approximately 8:30am-5:00pm and will not be scheduled for on-call shifts. We anticipate you will be scheduled to work 100% at the Hyde Park campus. The days of the week that you are scheduled will be Monday-Friday. You will not be scheduled to work on weekends.
