This job is field based however you can work from home, and you'll be provided office space in our Los Angeles office.

Responsibilities

• Assess Veterans through comprehensive psychosocial evaluations to determine casemanagement needs, recovery goals and provide referrals to appropriate treatment or services.
• Work with the Veterans to obtain housing voucher from the PHA, provide housing search assistance, facilitate smooth movein and ongoing case management to ensure housing retention.
• Employ Motivational Interviewing and Harm Reduction approaches to promote Veteran followthrough with referrals for: preventive care and treatment of medical conditions, substance use and dependence, other mental health conditions, and problematic health behaviors.
• Conduct regularly scheduled home visits to assess Veteran's housing stability, social connection, recovery, and ensure residence is safe and in compliance with local housing codes.
• Act as a liaison with VA and community resources, landlords, and PHA.
• Assist Veterans to identify housing goals, develop skills that increase selfsufficiency and eliminate barriers.
• Collect and submit HUD-VASH program evaluation data on Veteran participants.
• Identify opportunities to collaborative with local apartment complexes, rental agencies, and community services.
• Preform quality assurance checks on a quarterly basis.
• Attend agency meetings and participate in community partner meetings as a Madison Avenue Support Services representative.

Travel Required
You may be expected to travel within your SPA for this position.

Pay:
$40.85 per hour
