- Develop, implement and oversee individualized, strength-based family focused plans for each client, including a crisis/safety plan that supports the client, family and community
- Provide supervision of community-based clients on various levels of probation and coordinate appropriate treatment services particularly those court ordered and supported by assessments
- Maintenance of case files, physical and electronic, specific to County requirements
- Liaison with treatment providers and youth while in out of home care or community services to monitor adequacy of care, client progress and family involvement
- Demonstrate ability to be culturally competent and ensure treatment services for clients are conducted in such a manner
- Respond to all crisis and risk incidents within the required time frames and employ the crisis plan as needed
- Prepare and convey accurate client progress to the 3rd Judicial Circuit Court through competent court report compilation and verbal testimony
- Adherence to 3rd Judicial Circuit Court policies pertaining to adjudicated clients assigned to the agency including compliance with court ordered requirements
- Document electronically accurate and timely case notes regarding client progress through contacts with clients, providers, parents and other members of the treatment team
- Conduct in person meetings with members of the treatment team for each client within determined time parameters, including therapists, parents, clients, and others as determined
- Completes initial and ongoing agency training as required including building safety, policies and procedure, GWI custom training and DWC topics
- Attend and participate in Care Management Organization in-services and staff development trainings, including obtaining non-violent crisis intervention certification (CPI)
- Minimum Bachelor's Degree in education, social work, criminal justice, psychology or related human services
- Two years' experience working with at risk populations preferred
- Previous juvenile justice residential and/or detention experience preferred
- Familiarity and experience with screening and assessment tools preferred
- Computer skills, including experience with industry standard e-mail, word processing and database software
- Organizational ability, flexibility, resourcefulness, a strong work ethic, and the ability to work independently and meet deadlines
- Ability to form and maintain positive working relationships with supervisor, co-workers, participants and community stakeholders and employers
- Knowledge of community resources
- Time management and organizational skills, the ability to work on multiple tasks
- Ability to implement crisis intervention techniques as necessary
- Ability to work effectively with a diverse population
- Valid driver's license, good driving record, reliable transportation and automobile insurance meeting minimum requirements as defined by the agency:
- Successful completion of initial and on-going employment screens, including, but not limited to criminal background, sex offender, drug screen, valid Michigan driver's license, abuse and neglect and vehicle record check as required
- Perform other job-related duties as assigned
- Must be able to complete manual tasks involving dexterity to enter and retrieve data using computer systems, systems applications, and other office equipment. Ability to walk, stand, sit for short or long periods of time in an office setting.
- In certain situations, may be required to work within changing physical environments due to support of child/family's needs.
- Ability to lift and transport materials/supplies up to 25 lbs. for internal documentation and recordkeeping, court support, and other administrative tasks.
- Full-time, 40 hours/week exempt employee. Must maintain a flexible schedule as needed.

The primary role of the Care Management Organization (CMO) Juvenile Probation Case Manager position is to monitor, coordinate and support treatment services for adjudicated youth assigned for supervision to Western Wayne Care Management Organization (WWCMO) as outlined in the contract with Wayne County 3rd Judicial Court and Wayne County Health, Human and Veteran Services. This may include but is not limited to client contact, documentation of client progress, court preparation and participation.
