Description

Southwest Human Development is seeking a Community Health Nurse for the High Risk Perinatal Newborn Intensive Care Program (HRPP/NICP) working in the Southwest area of Maricopa County.

The HRPP/NICP provides in-home community health nursing services to assist families and their newborns during the transition from the Newborn Intensive Care Unit to their home and community.

This position can be full time, part time or contract.


As a Community Health Nurse, you will work in collaboration with families and other community providers, complete developmental screenings, provide support and education on development, infant and home safety and health prevention/promotion.

You will attend regular team meetings, case conferences and have regularly scheduled reflective supervision time to support you in your work.


Primary Duties:


  • Identifies highrisk infants and children through hospital (Levels II and Level lII) and community referrals from Maricopa County.
  • Participates in/facilitates the exchange of information from hospital to nurse to program family.
  • Facilitates exchange of referrals to appropriate nurse.
  • Explains the benefits and responsibilities of the followup program to families in a manner which is sensitive and responsive to individual family needs.
  • Works in collaboration with families to assess their educational, social, health and economic needs, develop an intervention plan and assist them in attaining their established goals.
  • Provides ongoing assessments using appropriate assessment tools, observation techniques and listening skills.
  • Promotes family selfsufficiency related to health issues, and measures desired participant and family outcomes.
  • Provides verbal and written information to families to promote/support
  • Increased understanding of growth and development norms.
  • Increased knowledge of health promotion practices (including prevention).
  • Importance of parentchild interaction that is age and developmentally appropriate.
  • Identifies children with physical and/or developmental problems and provides appropriate referrals for evaluation and intervention.
  • Identifies resource inadequacies, makes appropriate referrals for service and follows up to ensure that services are received.
  • Develops and maintains a professional, nonjudgmental, mutually respectful relationship that supports confidentiality and respects the privacy of individual families.

Experience:
Required

  • 2 year(s): Minimum of two (2) years experience in Neonatal, Maternal/Child health, Pediatrics, Community Health, and/or Home Visiting is required.

Education:
Required

  • Bachelors or better in Nursing

Licenses & Certifications:
Required

  • Driver's License
  • Registered Nurse
  • Fingerprint Card Level 1
Preferred

  • CPR and First Aid Cert

Skills:
Preferred

  • Spanish, Bilingual
