Position Summary:
The purpose of this position is to provide services and enhance the lives of the individuals we serve. All work should reflect the mission and vision of the Progress Center.

The In Home Support Services Coordinator provides oversight of the In Home Support Services program, reports to the In Home Support Manager or Manager of Residential Services, in addition to working direct care hours with a client or clients on a weekly basis.

Duties:

Program Duties:

  • Respond to vendor calls and meet potential service recipients, case manager and families.
  • Write Service Implementation Plans
  • Attend PCP meetings for individuals served
  • Oversee service schedule
  • Supervise DSP staff, providing them with training and support
  • Provide Direct Support Coverage as needed.
  • Facilitate ongoing communication with case managers, and other involved parties.
  • Attend Leadership and other agency meetings as requested.
  • Comply with any agency or staff programmatic changes.

Staff Supervision:

  • Initiate and facilitate onsite staff supervisory meetings
  • Facilitate individual and group supervisions
  • Oversee In Home Support staff schedules.
  • Write and conduct staff performance evaluations.

Direct Care:

  • Provide inhome support services to adults in home and community settings with particular emphasis on Independent Living skills, Activities of Daily Living, Self
  • Advocacy, as well as, supporting the individual with social activities to reduce risk of isolation and increase opportunities for unpaid relationships.
  • Follow the Individual Service Plan and accurately document services provided to the individual

General:

  • Comply with all federal, state and local regulations as regards services for persons with developmental or intellectual disabilities or autism, including rights of recipients and mandatory reporting.
  • Comply with Office of Aging and Disability Services and Department of Human Services regulations.
  • Demonstrate respect and add value to the lives of those we support
  • Fulfill all training requirements
  • Participate as a member of the Team

Qualifications:

  • High School Diploma or equivalent
  • Two years' experience in the field.
  • Two years' supervisory experience.

Abilities:

This position requires the ability to function comfortably both independently and in a team in environment, as well as the ability to effectively communicate with staff at all levels.

Exceptional organization, communication and interpersonal skills are essential. Must have supervisory experience. Attention to detail is imperative for the success of this position. Proficiency with electronic software (Microsoft Office Suite) and electronic health records. Strong writing, typing and organizational skills are necessary. Strong interpersonal skills, and a professional presentation are required.


Physical Demands:

The physical demands described herein are those that must be met by an employee to successfully perform the essential functions of the job and include climbing stairs regularly, carrying and lifting materials weighing up to 25 lbs, bending and stretching.


Mental Demands:

Must be able to assume a wide range of responsibilities, be self-directed, and effectively work within a busy and demanding environment.

The Progress Center employees may have access to or be required, as part of the duties of this job, to view, use, and/or be aware of certain Protected Health Information (PHI) or other facts about individuals that are of a highly personal and confidential nature.

Employees must follow and adhere to all HIPAA and agency confidentiality and privacy policies and practices. Employees may also be required to follow additional confidentiality policies and practices required by certain programs or business units.


Work Environment:

The In Home Supports Services Coordinator will primarily work from The Progress Center Campus at 5 Fore Street, Oxford location with potential travel to other locations.


Job Performance Evaluation:
The employee will be evaluated at 90 days and annually thereafter unless performance warrants more frequent evaluations.


Employment Conditions:
This is a full time non-exempt position with those related benefits as outlined in the personnel policies.

An initial background check will be completed to ensure the employee meets the standards set by the agency and random background checks will be conducted throughout employment to ensure continued integrity of the employee.


Pay:
$18.37 to $21.51
