Under the direction of the Hospital Site Manager, the Social Services Worker provides direct service to clients and their families.

This includes assessment and counseling therapy services, and seeking resources to answer unmet needs.

The Social Services Worker works collaboratively with the interdisciplinary treatment to assist with admission, coordinate care and discharge planning for patients, their family members, clinics, outpatient providers and treatment teams, and/or other referral sources.


ESSENTIAL RESPONSIBILITIES

  • Collaborates with the Nursing Staff and Provider to develop and implement an individualized personcentered treatment plan that is appropriate for brief intervention. Completes the signed treatment plan and reviews with each patient and any other involved parties.
  • Completes all social work assessments for patients including but not limited to psychosocial, ASAM, PRAPARE, safety plans.
  • Participate in clinical staffing providing relevant social work information to the interdisciplinary treatment team.
  • Gathers information from the patient and involved parties to provide a more thorough assessment of what the patient's immediate needs are for treatment and discharge.
  • Actively coordinates care for patients operating with accuracy, compassion and efficiency to ensure services are coordinated with facility staff and outpatient teams/providers. Identifies and addresses any gaps in service needs including social determinants of health (SDOH) for participating parties in service delivery to the patient and makes appropriate recommendations to meet patient's needs at discharge.
  • Collaborates with the interdisciplinary treatment team to engage, monitor and communicate with the patient ensuring safety is maintained while meeting the individual's needs
  • Facilitates individual and group therapy services for patients.
  • Demonstrates effective engagement skills; inspires hope and promotes recovery and resiliency providing education and community resources based on the intervention provided
  • Demonstrates effective documentation skills, ensures all interactions are documented in the designated electronic medical record in accordance with policies, procedures, and industry and/or organizational standards of practice.
  • Coordinates and facilitates Adult Recovery Team meetings for patients and their formal and informal support systems
  • Models appropriate physical and emotional boundaries with clients and coworkers.
  • Provides emotional support to help clients manage negative symptoms and utilize coping strategies to enhance their quality of life and daily functioning.
  • Maintains a working knowledge of the Title 36 Involuntary Commitment process completing prepetition screening forms completely and thoroughly as necessary.
  • Coordinates and at times facilitate hospital visitation for patients and their support systems.
  • Completes aftercare phone calls to patients discharging from the inpatient units.
  • Intervenes proactively with clients in situations where clients may be escalating emotionally and/or behaviorally using approved nonviolent crisis intervention techniques, including the least restrictive interventions appropriate for the situation.
  • Attends and participates in shift reports, site meetings, and trainings as mandated by agency policy and procedure
  • Assists with utilization management and review calls as needed for patient review and continued stay.
  • Communicates effectively and promotes quality service within a compassionate environment by treating clients and coworkers with respect and dignity
  • Helps to maintain a clean and orderly environment in all client, staff and facility areas
  • Supports a work climate that emphasizes collective team performance and demonstrates Polara Health's core competencies and values.
  • Other duties as assigned

EDUCATION AND EXPERIENCE

  • 21 years of age or older
  • Minimum qualifications: A Master's degree from an accredited College or University in Social Work, Counseling, Marriage and Family Therapy, Addiction Counseling. Two years previous social services experience.

Preferred Qualifications: 4 Years clinical experience, and/or Independent or Associate licensure in related field; knowledge and experience with AZ Title-36 legal process, Previous experience in hospital setting or behavioral health clinic.
