- This position is responsible for but is not limited to conducting diagnostic assessment, outpatient therapy, crisis intervention, Community Support Services, Therapy/Counseling and other services.
- The clinician utilizes generally accepted therapeutic interventions that are researched/evidence-based or best practice models and permissible under the CMS and other funding sources guidelines.
- The clinician conducts treatment planning, participates in team meetings and FSOH required professional development trainings, completes all documentation requirements according to FSOH standard operating procedures including timelines for completion.
- The clinician works collaboratively with the client's primary support system, other care providers and community agencies to facilitate and achieve treatment goals and objectives. Efforts are centered on client needs and preferences, and conducted in a culturally-sensitive way. All activities must be clinically/medically necessary and legally permissible according to applicable rules and regulations.
- Conducts person-centered, culturally sensitive and trauma informed clinical evaluation. Gathers relevant current and historical clinical information including lethality and propensity for violence. Evaluates the client's mental status. Reviews client's strengths, needs, abilities, and preferences. Assesses the functioning level including risk and protective factors. Determines the most appropriate level and intensity of service(s)/treatment that is medically necessary and/or the need for referral and further assessment. Formulates a diagnostic impression using the Diagnostic & Statistical Manual of Mental Disorders.
- Develops a treatment plan with the client and as appropriate with significant others. Collaborates with the FSOH treatment team, external care providers and other serving agencies.
- Conducts therapy, crisis intervention, Community Support, Therapy/Counseling services and other duties as assigned.
- Responds to client's clinical needs in a timely, responsive way.
- Maintains appropriate documentation of services delivered and all other documentations as required by FSOH in order to comply with applicable rules and regulations. Submits all paperwork on a timely basis. Documentations are complete and meet the applicable standards.
