    We are currently seeking a Care Manager Assistant to join the Home Care Program team at the Agency on Aging of South-Central Connecticut. As a Care Manager Assistant, you will play a crucial role in supporting our mission to serve clients in the community and ensure the well-being of individuals with disabilities and frail older adults who wish to remain safely in their own homes.

    Responsibilities:

    • Conduct 400 monthly/quarterly reviews of current clients per month (up to 350 calls per month if responsible for conducting client satisfaction surveys), via phone contact or home visit and document all client results in the chart using the appropriate documentation guidelines.
    • Provide follow-up where indicated upon completion of a monthly/quarterly contact, including confirming hospital/SNF admission and/or discharge dates, making referrals for services, and assisting in monitoring and adjustment of client care plans with care manager's approval.
    • Assist pending clients access CHCPE by making at least monthly contact to facilitate the funding source application process.
    • Manage the DSS Memo Queue daily by reading memos, determining care manager assigned and notifying care manager and their supervisor to read the memo using task delegation in NOMMA client management system.
    • Provide clerical support to care managers and coordinators.

    Participate as a member of a work group and function as back-up to all other care manager assistants in managing the DSS Memo Queue and other duties as assigned.

    Qualifications:

    • High school diploma or general education degree (GED); Associate degree preferred; or two to five years related experience and/or training; or equivalent combination of education and experience.
    • Bachelor's degree in social work, gerontology, or related field preferred.
    • Strong communication and organizational skills.
    • Compassionate and empathetic approach to working with older adults.
    • Proficiency in Microsoft Office Suite.
