Community Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.

CHC health care services are designed to meet the specific needs of the entire family - from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care.

We seek a Patient Care Coordinator to join our dynamic team in the Asbury Park Community Health Center supporting the LGBTQ and Ryan White Programs._**The Patient Care Coordinator's overall responsibilities include conducting care navigation activities to include but may not be limited to greeting and screening patients referred for case management services, identifying community resources, educating patients to case management services, participating as a member of the care team, providing information and referral services, providing advocacy and health center recordkeeping.

This position will administratively coordinate referrals requested by the RN Care Managers and other program staff as appropriate to conduct referral output, referral input from community health and social service resources, referral surveillance/tracking and closure.

This position requires the ability to work with low-income populations, meet health center goals and objectives, exercise good judgment, and facilitate administrative navigation of specialty, consulting and medical neighborhood services.

Principal Responsibilities:

  • Greet patients seeking services; administers initial patient screening questionnaire; provides case management program information; schedule external physician appointments as requested and appropriate;
  • Responds to patient requests for assistance by assessing service needs and providing information, referral and appointment services as appropriate. Followup on referrals and appointments in order to assess outcomes and provide additional services as required;
  • Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety, including ensuring complete and accurate patient demographic and current program eligibility information;
  • Assist patients with health care insurance enrollment, staying uptodate with insurance enrollment training and requirements, as well as insurance plans, copays, and premiums to educate patients of options;
  • Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services, or prescription assistance);
  • Identify and establish relationships with identified health and social service providers and maintain and utilize those cultural and community resources;
  • Maintain close relationships and liaisons with local service organizations in the service area;
  • Use appropriate technology tools to accomplish job functions; understand and utilize available technology as customer service, communication and data gathering tools as appropriate;
  • Support RN Care Managers in managing health and social needs of patients;
  • Coordinates with clinical care team to assist patients with durable medical equipment and related testing needs;
  • Adheres to the organization's policy in regards to professional behavior, appearance, and communication as a representative of VNACJ CHC;

Qualifications:

Job Competencies & Minimum Qualifications:


  • Minimum of high school graduate or equivalent. Medical Assistant, Certified Nurse Assistant or Community Health Worker, preferred;
  • Experience in community health or in another position with the provision of health and/or social services, supportive services, personal care, or related duties preferred;
  • Excellent techniques in dealing with a variety of individuals from various socioeconomic, ethnic and cultural backgrounds, in person and over the telephone;
  • Interviewing and assessment techniques;
  • Ability to motivate patients to fully participate in the case management program;
  • Knowledge of the targeted patient population;
  • Current and valid driver's license, auto insurance and registered vehicle

Working Conditions/Physical Demand:

Business Office Environment within a fast-paced healthcare center requiring superior customer service, exceptional phone etiquette and independent computer use.

Requires travel to different health center and community meeting locations.
