Case Manager
The Lamb Center (TLC) is a daytime drop-in shelter for individuals experiencing homelessness in Fairfax, VA.

It provides various services without cost to its guests, including breakfast, lunch, showers, laundry service, Bible study, case management, AA meetings, a nurse practitioner clinic, a dental clinic, workforce development, and much more.

TLC partners with local groups and organizations that have a passion for serving homeless individuals.


About the Case Manager Position
This person must fully support the mission, vision, and values of the Lamb Center.


Purpose


To assist our guests in navigating the homeless and social services system to meet their primary needs of housing stability, income, and physical and mental healthcare and substance use disorder treatment.


Case Manager Roles and Responsibilities
Under the supervision of the Director of Case Management:

  • Conduct intake interviews, assess needs and goals, and develop service plans which include goals, action steps, and identification of strengths and potential barriers
  • Provide support and accountability to the service plan
  • Coordinate with partner agencies in the Continuum of Care (CoC) and support the individual's access to available community resources. For example, coordinate with the Homeless Healthcare Program, Medical Respite Program, hospitals, Community Services Board, mental health facilities and other healthcare providers including Medicaid. Support court/probation compliance
  • Maintain case and Homeless Management Information System (HMIS) files that meet agency and CoC standards. Maintain uptodate case load status on the agency By Name List in accordance with CoC standard practice
  • Document homeless history and complete referrals and paperwork needed for the Coordinated Entry Prioritization Pool, various forms of Vouchers and Permanent Supportive Housing programs
  • Deescalate crisis situations with guests. Connect to appropriate emergency services and/or refer to treatment
  • Facilitate the restoration of the individual's natural supports where appropriate.
  • Represent the interests of our guests at relevant CoC and multidisciplinary meetings. Attend internal and external trainings as required to stay abreast of agency program developments and best practices. Participate in CoC initiatives and committees
  • Support formerly homeless/newly housed individuals' ability to maintain housing by assisting them with adhering to lease terms, ensuring compliance with community rules, rent and utility payments, etc. Advocate for, and support individuals in selfadvocacy related to leasing, fair housing, and reasonable accommodations
  • Other duties as assigned

Qualifications for Case Manager

  • Master of Social Work highly preferred
  • 3+ years of case management experience in homeless services or related field serving highneed individuals, including serious mental illness, substance use disorder, and cooccurring disorders
  • Bilingual English/Spanish preferred
  • Knowledge of and commitment to Housing First principles, traumainformed care, case management best practices, and Diversity, Equity, Inclusion, and Belonging
  • Knowledge of Fairfax County Continuum of Care, homeless services, and social service resources is a plus
  • Computer skills: basic office automation skills are required, including willingness and ability to learn enduser functions in the Fairfax County Homeless Management Information System, are required. More advanced skills, including ability to learn agency administrator functions, are a plus.
  • Strong written and oral communication skills

Working Conditions and Physical Requirements

  • Ability to sit or stand for long periods
  • Ability to lift items weighing 1020 pounds
  • Ability to operate standard office equipment and keyboards
  • Exposure to loud, hectic environment
  • Possible contact with individuals in crisis who may not be attentive to basic personal hygiene, health, and safety practices

$61, $63,500.00 per year
